What are underactive psychiatric emergencies?
Underactive psychiatric emergencies are acute mental health crises characterized by psychomotor retardation, a condition where both physical movements and cognitive processes slow down significantly. Unlike overactive emergencies that involve agitation or violence, these emergencies present with diminished activity, mutism, and sometimes complete immobility. Psychomotor retardation involves slowed thought processes and reduced physical movements, manifesting as decreased speech, slumped posture, and extreme fatigue.
These emergencies most commonly occur in two major psychiatric conditions: severe depression and catatonic states. The individual becomes dependent on others for all their needs as they become immobile and tend to remain in a vegetative-like state, though their consciousness often remains intact.
Understanding psychomotor retardation in depression
In severe depressive episodes, particularly depressive stupor, patients experience profound psychomotor retardation. This central feature of depression includes both motor and cognitive impairments that significantly affect functioning. Patients may appear frozen, speak in monosyllables if at all, and show minimal response to their environment.
What makes this particularly challenging is that depression presents with sadness, emptiness, or irritable mood accompanied by somatic and cognitive changes that can progress from mild slowing to complete immobility. The condition develops gradually, sometimes over weeks, with symptoms worsening in the morning. Unlike simple fatigue, this represents a genuine inability to initiate movement or thought processes.

Extreme slowness: Every action takes considerably longer than normal, from simple tasks like getting out of bed to responding to questions.

Airway assessment and protection
Check for patency: Ensure the airway is clear and unobstructed. In stuporous states, patients may have decreased protective reflexes.
Position properly: Place patients in positions that prevent aspiration and maintain airway patency.
Monitor swallowing: Assess the gag reflex and ability to handle secretions safely.
Breathing evaluation

Maintaining basic physiological needs
Nutrition and hydration: Patients may require assistance with feeding or tube feeding if unable to take oral nutrition. Monitor intake carefully and maintain accurate fluid balance charts.
Elimination: Assist with toileting needs, monitor bowel and bladder function, and prevent urinary retention or constipation.
Hygiene: Provide complete care for bathing, oral care, and grooming to maintain dignity and prevent infections.
Skin integrity: Turn immobile patients regularly and use pressure-relieving devices to prevent pressure ulcers.
Safety monitoring
