Malaria remains one of India’s most significant public health challenges, particularly in tribal and rural areas. Despite remarkable progress in recent years, the disease continues to affect millions of people annually, making effective prevention and control strategies at every level of healthcare essential. From the household to district hospitals, a coordinated approach involving individuals, community health workers, and medical professionals forms the backbone of India’s malaria elimination efforts.

Understanding malaria and its burden in India
Malaria is a life-threatening disease caused by Plasmodium parasites transmitted through the bites of infected female Anopheles mosquitoes. In India, two species pose the greatest threat: Plasmodium falciparum, the deadlier variant that can cause cerebral malaria, and Plasmodium vivax, the more prevalent species across the country.
The National Center for Vector Borne Diseases Control (NCVBDC), formerly NVBDCP, serves as the central nodal agency for malaria prevention and control in India. According to recent data, India contributed approximately 83% of estimated malaria cases in the WHO Southeast Asia region. However, the country has achieved significant reductions, with reported cases dropping by approximately 85% between 2016 and 2021 following the launch of the National Framework for Malaria Elimination.
Malaria is prevalent across 747 districts in India, putting approximately 698 million people at risk. India has set an ambitious target to eliminate malaria by 2030, requiring sustained efforts at every level of the healthcare system.
Recognizing malaria symptoms early
Early recognition of malaria symptoms is critical for preventing severe complications and death. The most common early symptoms include fever, headache, and chills, typically appearing within 10-15 days of being bitten by an infected mosquito. Because these symptoms can be mild and resemble other febrile illnesses, prompt testing is crucial.
Warning signs of severe malaria
Certain symptoms indicate progression to severe malaria and require immediate medical attention. These include extreme tiredness and fatigue, impaired consciousness, multiple convulsions, difficulty breathing, dark or bloody urine, jaundice (yellowing of eyes and skin), and abnormal bleeding. P. falciparum malaria can progress to severe illness and death within 24 hours if left untreated, making early diagnosis and treatment particularly important.
High-risk populations
Certain groups face higher risks of severe malaria infection: infants and children under five years, pregnant women and girls, travellers to endemic areas, and people living with HIV or AIDS. In Africa, children under five account for about 75% of all malaria deaths. Similar patterns exist in India’s high-endemic regions, making targeted interventions for these groups essential.
Prevention strategies at the household level
Effective malaria prevention begins at home. Individuals and families in endemic areas can significantly reduce their risk through several practical measures.
Personal protection measures
Using mosquito nets while sleeping remains one of the most effective prevention methods. Insecticide-treated nets (ITNs) are particularly effective and are distributed through government programmes. Other protective measures include using mosquito repellents containing DEET, IR3535, or Icaridin after dusk, wearing protective clothing, and installing window screens.
Environmental management at home
Reducing mosquito breeding sites around the home is equally important. Anopheles mosquitoes breed in stagnant water, so eliminating standing water in containers, discarded tyres, flower pots, and drainage areas can significantly reduce local mosquito populations. Covering water storage containers and maintaining proper drainage around homes are simple but effective measures.
Community-level interventions and the role of ASHA workers
Community-level interventions form the cornerstone of India’s malaria control programme. The Accredited Social Health Activists (ASHAs) play a crucial role as the first point of contact between communities and the public health system.
ASHA workers as frontline providers
ASHAs are trained female community health workers who provide essential health services at the village level. Each ASHA typically covers a population of about 1,000 people. Since 2010, ASHAs have been progressively involved in malaria control activities, functioning as fever treatment depots in endemic areas. They are trained to diagnose malaria using Rapid Diagnostic Tests (RDTs), provide treatment with artemisinin-based combination therapy (ACT) as per national guidelines, and report cases to the health system.
The Malaria Elimination Demonstration Project in Mandla district, Madhya Pradesh, demonstrated how intensive ASHA training can significantly improve malaria diagnosis and treatment outcomes. Studies showed that periodic training and capacity building efforts resulted in substantial improvement in ASHAs’ knowledge and practices related to malaria management.
Community awareness and behaviour change
Community health workers also conduct awareness campaigns about malaria prevention, symptoms, and the importance of seeking early treatment. Behaviour change communication (BCC) activities help communities understand how malaria spreads, the importance of using bed nets, and when to seek medical care. Door-to-door campaigns and interpersonal communication are the primary strategies used for community outreach.
Vector control at the community level
Vector control is vital for malaria prevention and remains a core component of elimination strategies. Two primary interventions are implemented at the community level.
Indoor residual spraying
Indoor Residual Spraying (IRS) involves applying insecticide to interior surfaces of homes where mosquitoes rest. In India, IRS is conducted twice yearly during transmission season in high-risk areas. The programme uses approved insecticides following WHO guidelines, though the emergence of insecticide resistance among mosquitoes presents ongoing challenges requiring new approaches.
Long-lasting insecticidal nets distribution
Long-Lasting Insecticidal Nets (LLINs) are distributed through mass campaigns in high-endemic areas. These nets remain effective for several years and provide protection for families while sleeping. The NCVBDC coordinates LLIN distribution across states, targeting areas with high annual parasite index (API). Communities are advised to continue using their nets even beyond the three-year expected lifespan until replacement nets become available.
Health facility-level interventions
Health facilities at various levels play distinct roles in malaria control, from primary health centres to district hospitals.
Diagnosis and treatment at primary health centres
Primary Health Centres (PHCs) serve as the first institutional level of contact for confirmed malaria cases. Laboratory technicians at PHCs examine blood slides for microscopic confirmation of malaria parasites. WHO recommends that all suspected malaria cases be confirmed through diagnostic testing, either microscopy or rapid diagnostic tests, before treatment.
Treatment protocols follow national guidelines, with artemisinin-based combination therapy (ACT) as the first-line treatment for P. falciparum malaria. For P. vivax infections, chloroquine remains effective in most areas, with primaquine added to prevent relapses.
Referral services for severe cases
District hospitals and Sub-Divisional Hospitals provide care for severe and complicated malaria cases. These facilities have sentinel surveillance systems to monitor malaria trends and manage cases requiring hospitalization. Artesunate injections are available at referral hospitals for treating complicated malaria cases, including cerebral malaria.
Training and supervision
Health facilities also serve as training centres for community health workers. Regular training programmes for ASHAs, Auxiliary Nurse Midwives (ANMs), and multipurpose health workers are conducted at district and block levels. These trainings cover diagnostic techniques, treatment protocols, and reporting requirements.
Preventive treatment for high-risk groups
Preventive chemotherapy is an important strategy for protecting vulnerable populations. Preventive treatment involves administering antimalarial medicines to high-risk groups during periods of greatest malaria risk, regardless of whether they are currently infected.
Intermittent preventive treatment during pregnancy (IPTp) protects pregnant women and their unborn children from the consequences of malaria infection, including premature delivery and low birth weight. Mass drug administration may be implemented in specific high-burden areas as part of elimination efforts.
Indicators for measuring programme effectiveness
Monitoring and evaluation are essential for assessing malaria control programme success. Key indicators include community awareness about malaria transmission and prevention, proportion of fever cases tested for malaria, treatment coverage among confirmed cases, reduction in malaria-related morbidity and mortality, and API trends across districts.
The Global Fund and other partners support surveillance systems that track these indicators. Electronic health management information systems help districts monitor case trends and respond promptly to outbreaks. The National Anti-Malaria Management Information System (NAMMIS) enables web-based reporting from district to national levels.
Challenges and the path forward
Despite significant progress, challenges remain. Antimalarial drug resistance is an emerging concern, with partial artemisinin resistance confirmed in several countries. Insecticide resistance among mosquito vectors requires continuous monitoring and the introduction of new vector control products. Ensuring consistent supply of diagnostics and medicines at the community level, particularly in remote areas, remains an operational challenge.
India’s commitment to malaria elimination by 2030 requires sustained investment, strengthened surveillance, community engagement, and continued research into new tools. The integration of malaria services with primary healthcare strengthens overall health system capacity while moving toward elimination goals.
