Understanding high blood pressure
High blood pressure, or hypertension, occurs when the pressure in blood vessels is too high, typically defined as readings of 140/90 mmHg or higher. According to the WHO, an estimated 1.4 billion adults aged 30-79 years worldwide had hypertension in 2024, representing about 33% of this population. Alarmingly, approximately 600 million adults with hypertension remain unaware they have the condition.
The challenge with hypertension is its silent nature. Most people with high blood pressure experience no symptoms, which is why regular screening is crucial. When symptoms do occur with very high blood pressure (usually 180/120 or higher), they may include severe headaches, chest pain, dizziness, difficulty breathing, blurred vision, and nosebleeds. These symptoms require immediate medical attention.
Risk factors and prevention
Several factors increase the risk of developing hypertension. Modifiable risk factors include unhealthy diets (particularly excessive salt intake), physical inactivity, tobacco use, excessive alcohol consumption, and being overweight. Non-modifiable factors include family history, age over 65 years, and conditions like diabetes or kidney disease.
Lifestyle modifications play a significant role in both preventing and managing high blood pressure. The American Heart Association recommends following a DASH-style eating pattern, staying physically active, reducing or eliminating alcohol, and practicing stress-reducing activities like meditation or yoga. For blood pressure management, adults should aim for less than 2,300 mg of sodium daily.
Treatment approaches
When lifestyle changes alone aren’t sufficient, medication becomes necessary. Common antihypertensive medications include ACE inhibitors (such as enalapril and lisinopril), angiotensin receptor blockers (ARBs), calcium channel blockers, and diuretics. The treatment goal for most adults is maintaining blood pressure below 140/90 mmHg, while those with cardiovascular disease, diabetes, or chronic kidney disease may need to achieve levels below 130/80 mmHg.
Adherence to prescribed medications is essential. Uncontrolled hypertension can lead to serious complications including heart attack, stroke, heart failure, and kidney damage. Regular monitoring and follow-up appointments help ensure treatment effectiveness.
Recognizing and responding to chest pain
Chest pain is one of the most concerning cardiovascular symptoms and often indicates underlying coronary artery disease (CAD). CAD, the most common type of heart disease, develops when cholesterol and other substances form plaque that narrows the coronary arteries, limiting blood flow to the heart muscle.
Types of chest pain
Angina pectoris is chest pain or discomfort that occurs when part of the heart doesn’t receive enough blood and oxygen. Stable angina typically appears during physical activity or emotional stress and resolves with rest or nitroglycerin medication. The NHS notes that symptoms usually pass in less than 10 minutes and can be relieved by resting or using a nitrate tablet or spray.
Unstable angina, however, represents a medical emergency. If angina symptoms change suddenly, become more severe, occur at rest, or don’t respond to nitroglycerin, immediate medical attention is essential. These changes may signal an impending heart attack.
Warning signs requiring urgent care
Heart attack symptoms differ from typical angina. According to the Cleveland Clinic, warning signs include pressure or discomfort that travels down the left arm, numbness that spreads to the neck, and pain that worsens with physical activity. Unlike angina, heart attack symptoms typically last longer than 15 minutes and aren’t relieved by rest or nitroglycerin.
Treatment for coronary artery disease combines lifestyle modifications with medical therapy. Every patient should receive guideline-directed medical therapy, which typically includes low-dose aspirin, beta-blockers, as-needed nitroglycerin, and statin medications. More severe cases may require interventional procedures like angioplasty with stenting or coronary artery bypass surgery.
Rheumatic heart disease: a preventable condition
Rheumatic heart disease (RHD) remains a significant health concern, particularly among children and young adults in low- and middle-income countries. According to the WHO, this serious yet preventable condition begins with a simple sore throat caused by Streptococcus pyogenes (group A streptococcus) bacteria. When strep infections go untreated, the immune system may react abnormally, attacking the body’s own tissues, including the heart valves.
Who is at risk?
Rheumatic fever, which leads to RHD, often occurs in children ages 5 to 15. Those who experience repeated strep throat infections face the highest risk. The condition is more prevalent where poverty is widespread and access to healthcare services is limited. Globally, rheumatic heart disease causes approximately 200,000 to 250,000 premature deaths every year and remains a major cause of cardiovascular death in young people.
Prevention and early intervention
The key to preventing rheumatic heart disease lies in promptly recognizing and treating strep throat with appropriate antibiotics. Antibiotic therapy has significantly reduced both the incidence and mortality rate of rheumatic fever and rheumatic heart disease.
For those who have already experienced rheumatic fever, secondary prevention is critical. Long-term antibiotic prophylaxis, typically with benzathine penicillin G given by injection every 3-4 weeks, helps prevent additional streptococcal infections that could cause further valve damage. Early referral to a cardiologist for ongoing monitoring is essential, as symptoms of heart valve damage may not appear until 10 to 20 years after the initial illness.
Anemia and cardiovascular health
While not traditionally classified as a cardiovascular condition, anemia significantly impacts heart function and overall cardiovascular health. Anemia is a condition in which there aren’t enough healthy red blood cells or the hemoglobin concentration is lower than normal, reducing oxygen delivery to organs and tissues.
Causes and prevalence
Globally, anemia affects approximately 40% of children aged 6-59 months, 37% of pregnant women, and 30% of women aged 15-49 years. The WHO has recognized iron deficiency anemia as the most common nutritional deficiency worldwide, affecting about 30% of the population.
The causes of anemia are diverse. Iron deficiency accounts for approximately half of all anemia cases and may result from inadequate dietary intake, decreased absorption, increased demand (such as during pregnancy), or blood loss. Other causes include infections like malaria, chronic diseases, heavy menstruation, and inherited conditions like thalassemia and sickle cell disease.
Symptoms and treatment
Common symptoms of anemia include fatigue, weakness, shortness of breath, and reduced physical work capacity. When anemia is severe, it forces the heart to work harder to deliver oxygen throughout the body, potentially contributing to cardiovascular complications.
Treatment varies based on the underlying cause. Iron supplements, usually ferrous sulfate, can help build up iron stores in the body. Taking supplements on an empty stomach with vitamin C enhances absorption. For those who cannot tolerate oral iron or have severe deficiency, intravenous iron therapy may be necessary.
Beyond supplementation, addressing root causes is crucial. This may involve treating infections, managing chronic conditions, or addressing dietary deficiencies. In areas where malaria or parasitic infections are endemic, treating these underlying conditions becomes part of comprehensive anemia management.
Taking action for heart health
Cardiovascular conditions, whether hypertension, coronary artery disease, rheumatic heart disease, or anemia, share common themes: early detection matters, lifestyle modifications help, and timely medical intervention saves lives. Regular health check-ups, awareness of warning signs, and prompt treatment of seemingly minor infections can prevent serious complications.
Healthcare providers, particularly nurses working in community settings, play a vital role in screening, education, and early referral. By understanding these conditions and their management, we can work toward reducing the global burden of cardiovascular disease.
