When plasma cells in the bone marrow begin multiplying abnormally, they can transform into a cancer called multiple myeloma. Unlike healthy plasma cells that produce antibodies to fight infections, these cancerous cells crowd out normal blood cells and produce abnormal proteins that accumulate throughout the body. For nursing professionals, understanding this complex condition is essential to providing comprehensive care that addresses both the physical complications and emotional challenges patients face.

Understanding multiple myeloma and its progression

Multiple myeloma develops when a single plasma cell in the bone marrow undergoes malignant transformation. These cancer cells multiply rapidly and produce monoclonal proteins, also called M proteins, which serve no useful purpose. As these abnormal cells accumulate, they disrupt normal blood cell production, leading to anemia, weakened immunity, and impaired clotting function.

The disease affects approximately 149,956 people in the United States, with about 7.1 new cases per 100,000 people annually. Most patients are diagnosed between ages 65 and 74, though the condition can occur in younger adults. Men face slightly higher risk than women, and the incidence is twice as high among Black individuals compared to other racial groups.

How the disease damages bones

One of multiple myeloma’s most devastating features is its impact on bone health. The cancer cells release substances that stimulate osteoclasts-cells responsible for breaking down bone tissue. This accelerated bone destruction leads to lytic lesions, pathological fractures, and severe bone pain, particularly in the spine, ribs, and hips. About 80% of patients experience bone complications at diagnosis.

Identifying risk factors

While the exact cause remains unknown, certain factors increase susceptibility to multiple myeloma. Age is the primary risk factor, with most diagnoses occurring after age 60. Having a family history of the disease, particularly in siblings or parents, raises risk levels. Previous diagnosis of monoclonal gammopathy of undetermined significance also increases likelihood of progression to active myeloma.

Environmental exposures may contribute to risk. Research suggests potential links to ionizing radiation and certain chemical exposures, including pesticides and herbicides, though these connections require further investigation.

Recognizing symptoms and establishing diagnosis

Early-stage multiple myeloma often produces no symptoms, making it challenging to detect. When symptoms appear, they typically include persistent bone pain, fatigue, frequent infections, and unexplained weight loss. The acronym CRAB helps identify key diagnostic criteria: elevated Calcium levels, Renal dysfunction, Anemia, and Bone disease.

Diagnostic procedures

Diagnosis requires multiple tests to confirm the presence and extent of disease. Blood tests reveal elevated M proteins, anemia, elevated calcium levels, and kidney dysfunction markers. A 24-hour urine collection detects Bence Jones proteins-light chain proteins that indicate myeloma. Bone marrow biopsy remains essential, showing abnormal plasma cell infiltration typically exceeding 10% of bone marrow cells.

Imaging studies identify bone damage. Skeletal surveys using X-rays or low-dose CT scans reveal lytic lesions, while MRI and PET scans help assess disease extent and detect soft tissue involvement.

Managing hypercalcemia complications

Hypercalcemia occurs in 10-15% of patients at diagnosis and affects more than 30% at some point during their disease course. As cancer cells destroy bone tissue, calcium floods into the bloodstream. Patients with hypercalcemia face significantly worse survival rates, with median survival of 40 months compared to 57 months in those without elevated calcium.

Nursing interventions for hypercalcemia

Immediate management focuses on aggressive hydration. Administer intravenous fluids to promote calcium excretion through the kidneys. Monitor intake and output meticulously, aiming for urine output of at least 100-150 mL per hour. Encourage oral fluid intake of 3-4 liters daily when patients can tolerate it.

Bisphosphonates form the cornerstone of pharmacological treatment. These medications inhibit osteoclast activity, reducing bone resorption. Common options include pamidronate and zoledronic acid administered intravenously. Monitor for potential complications including osteonecrosis of the jaw and renal toxicity.

Watch for signs of hypercalcemic crisis: confusion, severe dehydration, cardiac arrhythmias, and declining consciousness. These symptoms require immediate intervention. Implement fall precautions as neurological symptoms can impair balance and coordination.

Supporting bone health and managing pain

Bone pain affects most myeloma patients and requires comprehensive management strategies. Assess pain levels regularly using standardized scales. Document pain characteristics, location, and factors that worsen or relieve symptoms.

Position patients carefully to minimize stress on affected bones. Use pillows and supportive devices to maintain comfortable positions. Encourage gentle range-of-motion exercises to maintain mobility while avoiding activities that increase fracture risk. Teach patients proper body mechanics when moving or transferring.

Medication management

Administer prescribed analgesics on schedule rather than as needed to maintain consistent pain control. Options may include nonsteroidal anti-inflammatory drugs for mild pain, though these require caution due to potential kidney effects. Opioids may be necessary for moderate to severe pain. Monitor for side effects including constipation, which requires proactive management.

Bisphosphonates reduce skeletal complications and can decrease pain intensity. Studies show that regular bisphosphonate administration reduces pathological fractures, need for radiation therapy, and bone pain episodes.

Ensuring adequate hydration and preventing kidney damage

Kidney dysfunction develops in approximately 20-40% of myeloma patients. Light chain proteins, calcium overload, and dehydration all contribute to renal impairment. Maintaining adequate hydration becomes crucial to prevent kidney failure.

Encourage fluid intake of at least 3 liters daily unless contraindicated. Monitor blood urea nitrogen and creatinine levels to track kidney function. Watch for signs of declining renal function: decreased urine output, fluid retention, electrolyte imbalances, and rising creatinine levels. Educate patients about avoiding nephrotoxic medications, particularly nonsteroidal anti-inflammatory drugs.

Preventing and managing infections

Impaired immunity makes patients extremely vulnerable to infections. The abnormal plasma cells cannot produce functional antibodies, and treatment further suppresses immune function. Patients face 15 times higher infection risk compared to individuals without myeloma.

Teach meticulous hand hygiene to patients and caregivers. Recommend avoiding crowded places and contact with sick individuals. Ensure patients receive recommended vaccinations including pneumococcal, influenza, and shingles vaccines. Monitor for infection signs: fever above 100.5ยฐF, new cough, changes in urine characteristics, or wound drainage. Any suspected infection requires prompt evaluation and treatment.

Addressing anemia and fatigue

More than 60% of patients present with anemia at diagnosis. As cancer cells crowd out normal blood-forming cells, red blood cell production declines. Treatment can further worsen anemia. Assess patients for pallor, tachycardia, shortness of breath, and dizziness.

Monitor hemoglobin levels and complete blood counts regularly. Patients may require blood transfusions for severe anemia. Erythropoiesis-stimulating agents can boost red blood cell production in some cases. Encourage adequate nutrition with iron-rich foods. Help patients conserve energy by prioritizing activities and scheduling rest periods throughout the day.

Providing emotional support and education

A multiple myeloma diagnosis profoundly impacts patients emotionally. Many feel overwhelmed by complex treatment regimens and uncertain prognosis. Create an environment where patients feel comfortable expressing fears and concerns.

Provide clear, honest information about the disease and treatment options. Explain that while multiple myeloma cannot be cured, current treatments significantly extend survival and improve quality of life. Connect patients with support groups and counseling services. Involve family members in education sessions when appropriate.

Treatment adherence

Multiple myeloma treatment often involves complex medication regimens. Ensure patients understand when and how to take medications. Discuss potential side effects and management strategies. Provide written instructions and medication schedules. Emphasize the importance of completing prescribed courses even when feeling better.

Coordinating comprehensive care

Effective multiple myeloma management requires coordination among multiple healthcare providers. Maintain clear communication channels between physicians, nurses, pharmacists, physical therapists, and social workers. Document patient status, treatment responses, and any complications thoroughly.

Assess for treatment-related complications including peripheral neuropathy from certain chemotherapy agents, thromboembolism risk from immunomodulatory drugs, and cardiac dysfunction. Monitor vital signs and laboratory values according to treatment protocols. Report concerning changes promptly to the medical team.
