Fungal infections don’t always penetrate deep into the skin. Some fungi are content to live only in the outermost dead layers of our skin and hair. These are called surface mycoses, and while they might sound harmless, they can cause noticeable cosmetic changes that worry patients. Understanding these infections helps nursing professionals provide better patient education and recognize conditions that are often mistaken for more serious diseases.

What are surface mycoses?
Surface mycoses are fungal infections that affect only the outermost layer of the skin and its appendages like hair. Unlike deeper fungal infections, these organisms don’t invade living tissue. They colonize the stratum corneum (the dead outer layer of skin) and the hair shaft, feeding on keratin and other proteins found there. The three main types are tinea versicolor, tinea nigra, and piedra.
Tinea versicolor: The color-changing infection
Tinea versicolor is a common fungal infection caused by a yeast called Malassezia. This yeast is actually part of the normal skin flora in most people, but under certain conditions, it overgrows and causes visible symptoms.
Clinical presentation
The hallmark of tinea versicolor is patchy discoloration of the skin. Patients develop multiple oval or round patches that may be lighter or darker than the surrounding skin. The discoloration typically appears on the chest, back, shoulders, and upper arms. The patches often have fine scales that become more visible when the skin is gently scraped.
The name “versicolor” refers to the varied colors the patches can display. In people with lighter skin, the patches often appear brown or tan. In darker-skinned individuals, the patches may appear lighter than the surrounding skin, creating a hypopigmented appearance. This color variation becomes more noticeable after sun exposure, as the affected areas don’t tan normally.
Why does it occur?
Several factors increase the risk of developing tinea versicolor. Hot and humid weather promotes yeast overgrowth. People with oily skin are more susceptible. Hormonal changes, particularly during adolescence and young adulthood, can trigger the infection. This explains why teens and young adults are most commonly affected.
Diagnosis and treatment
Diagnosis is usually straightforward based on clinical appearance. A potassium hydroxide (KOH) preparation of skin scrapings reveals the characteristic “spaghetti and meatballs” pattern – short hyphae mixed with round yeast cells.
Treatment typically involves topical antifungal medications. Over-the-counter options include selenium sulfide shampoo, ketoconazole shampoo, or zinc pyrithione shampoo applied to affected areas. For widespread disease, oral antifungal medications may be prescribed. Patients should understand that while the infection clears quickly, the skin color may take several weeks to return to normal.
Tinea nigra: The dark palm stain
Tinea nigra is a much rarer surface mycosis that causes brown to black patches on the palms and soles. The causative organism is Hortaea werneckii, a pigmented fungus found in soil, decaying vegetation, and areas with high salt content.
Recognizing the infection
The typical presentation is a single, well-defined brown or black patch on the palm of one hand. The lesion is completely asymptomatic – no itching, no pain, no scaling. It grows slowly over weeks to months. The patch appears as if someone stained the skin with a dark dye, with the darkest pigmentation at the edges.
This infection is most common in tropical and subtropical regions, particularly in Central and South America, Africa, and Southeast Asia. In temperate climates, cases are rare and usually occur in travelers returning from endemic areas.
The diagnostic challenge
The main concern with tinea nigra is misdiagnosis. The dark patch can easily be mistaken for melanoma, a serious form of skin cancer. This makes accurate diagnosis critical. Research shows that dermatologists correctly identified tinea nigra in only 44% of cases initially, highlighting the importance of proper evaluation.
Diagnosis is confirmed through KOH preparation of skin scrapings, which reveals pigmented hyphae and yeast cells. The fungus can also be cultured on Sabouraud agar, where it grows as black, shiny colonies.
Treatment approach
Treatment is relatively simple. Topical antifungal creams containing azoles (ketoconazole, miconazole) or keratolytic agents (salicylic acid, Whitfield’s ointment) are typically effective. In some cases, simply scraping the lesion can remove the superficial infection. The condition usually resolves within two weeks of starting treatment and rarely recurs.
Piedra: Nodules on the hair shaft
Piedra is a surface mycosis affecting the hair shaft itself. The name comes from the Spanish word for “stone,” as the infection creates small, stone-like nodules attached to hair. There are two distinct types: white piedra and black piedra.
White piedra
White piedra is caused by Trichosporon species, particularly T. inkin and T. ovoides. The infection produces soft, white to light brown nodules that loosely attach to the hair shaft. These nodules are easiest to remove and typically affect facial hair, axillary hair, and pubic hair more commonly than scalp hair.
White piedra is more common in semitropical climates, including parts of Europe, the Middle East, southern United States, and Japan. The infection can lead to increased hair fragility and breakage if left untreated.
Black piedra
Black piedra is caused by Piedraia hortae and produces hard, firmly attached black or dark brown nodules on the hair shaft. This type is more common in tropical regions, particularly South America and Southeast Asia. Black piedra typically affects scalp hair and can cause significant hair damage due to the fungus’s strong keratolytic activity.
Clinical features and diagnosis
Patients with piedra notice small nodules encircling or attached to their hair shafts, ranging from less than 1 millimeter to several millimeters in diameter. The condition is otherwise asymptomatic – no itching or pain. A key clinical clue is a negative “hair pull test,” suggesting the problem is on the hair shaft rather than at the root.
Trichoscopy (examination of hair under magnification) helps distinguish piedra from other conditions like head lice. The gold standard for diagnosis is fungal culture of affected hair strands, which reveals characteristic hyphae and spores.
Treatment and prevention
The most effective treatment for piedra is shaving the affected hair. However, this may not be acceptable for social, cultural, or religious reasons. Topical antifungal agents offer an alternative. For white piedra, options include ketoconazole shampoo, selenium sulfide, or ciclopirox lotion. Black piedra may require oral terbinafine for complete clearance.
Prevention focuses on good personal hygiene. Regular hair washing, avoiding prolonged use of hair oils, and not covering wet hair are important measures. Patients should discard contaminated hair brushes and pillowcases to prevent reinfection.
Key nursing considerations
When caring for patients with surface mycoses, nurses should emphasize several important points. These infections are typically chronic and may persist for months or years without treatment. They are generally not contagious through casual contact, though sharing personal items like combs or towels should be avoided.
Patient education is crucial. Many patients worry that discolored skin or dark patches indicate something serious. Reassuring them that these are superficial infections that respond well to treatment can reduce anxiety. For tinea versicolor, setting realistic expectations about the time needed for skin color to normalize prevents disappointment.
Surface mycoses are often confused with other conditions. Tinea nigra can mimic melanoma. Piedra may be mistaken for head lice. White piedra can resemble bacterial infections of hair. Proper diagnosis prevents unnecessary treatments and ensures appropriate care.
