Candidiasis is one of the most common fungal infections you’ll encounter in clinical practice. Caused by an overgrowth of yeast from the Candida genus, this infection can affect multiple body sites-from the mouth and skin to the vagina and, in severe cases, internal organs. Understanding how candidiasis presents, who is at risk, and how to treat it effectively is essential knowledge for nursing professionals.

What is candidiasis?

Candidiasis is an opportunistic fungal infection caused by Candida species, most commonly Candida albicans. Interestingly, Candida yeast naturally lives on our skin and in various body parts including the mouth, throat, gut, and vagina. Under normal circumstances, it coexists harmlessly with other microorganisms. However, when the body’s natural balance is disrupted-whether through weakened immunity, antibiotic use, or other factors-Candida can multiply uncontrollably and cause infection.

While Candida albicans remains the most prevalent species (accounting for roughly 44% of infections), other species like Candida glabrata, Candida parapsilosis, Candida tropicalis, and Candida krusei can also cause infections. Recognizing the specific species matters because some are resistant to commonly used antifungal medications.

Types of candidiasis and their symptoms

Candidiasis manifests differently depending on which body site is affected. Here’s a breakdown of the main types:

Oral candidiasis (thrush)

Oral thrush is an infection of the mouth caused by Candida overgrowth. It’s more common in babies under one month old and older adults due to their lower immunity. It also frequently affects individuals taking antibiotics or corticosteroids, people with diabetes, and those with immunocompromising conditions like HIV/AIDS.

The characteristic symptoms of oral thrush include:

  • White patches on the tongue, inner cheeks, roof of the mouth, gums, and tonsils
  • Cottage cheese-like lesions that are slightly raised
  • Redness, burning, or soreness that may cause difficulty eating or swallowing
  • Slight bleeding if the patches are rubbed or scraped
  • Cracking at the corners of the mouth
  • Loss of taste or a cottony feeling in the mouth

In severe cases, particularly in immunocompromised patients, the infection may spread to the esophagus (Candida esophagitis), causing difficulty swallowing and a sensation of food being stuck in the throat.

Vaginal candidiasis

Vaginal yeast infections are among the most common fungal infections, affecting approximately 75% of women at least once in their lifetime, with 40-45% experiencing two or more episodes. These infections are more prevalent in women who are diabetic, pregnant, using hormonal birth control, or taking antibiotics.

Symptoms of vaginal candidiasis include:

  • Genital itching and irritation
  • Burning sensation, especially during urination or intercourse
  • White, thick discharge resembling cottage cheese
  • Redness and swelling of the vulva

Vaginal candidiasis can be classified as uncomplicated (mild to moderate, sporadic) or complicated (severe, recurrent, or occurring in patients with diabetes or weakened immunity). Complicated cases require longer treatment durations and special therapeutic considerations.

Cutaneous candidiasis (skin and nails)

When Candida infects the skin, it causes cutaneous candidiasis. This type of infection thrives in warm, moist areas where skin touches skin-such as the armpits, groin, beneath the breasts, and between fingers and toes. Diaper rash in infants is often caused by Candida.

Symptoms of skin candidiasis include:

  • Red, itchy rash with well-defined borders
  • Small pustules or bumps around the rash edges
  • Intense itching or burning
  • Moist, macerated skin in affected areas

Candida can also affect the nails (candidal onychomycosis), causing discoloration, thickening, brittleness, and sometimes nail detachment. Nail infections often occur in people whose hands are frequently in water or who have had poorly done manicures.

Invasive candidiasis

This is the most serious form of candidiasis and occurs when Candida enters the bloodstream (candidemia) or infects internal organs such as the kidneys, heart, brain, or bones. Invasive candidiasis primarily affects hospitalized patients and those who are severely ill, have central venous catheters, are undergoing chemotherapy, or have had recent surgery.

The symptoms are often nonspecific and include fever and chills that don’t respond to antibiotic treatment. Additional symptoms depend on which organ is affected. Candidemia is one of the most common bloodstream infections in the United States and carries an in-hospital mortality rate of approximately 30%.

Risk factors for candidiasis

Several factors increase the likelihood of developing a Candida infection:

Weakened immune system: People with HIV/AIDS, cancer, or those taking immunosuppressive medications are at higher risk. Extremes of age-newborns and elderly individuals-are also more vulnerable due to lower immunity.

Diabetes: Poorly controlled diabetes provides an environment conducive to Candida growth because elevated glucose levels in saliva and other body fluids promote yeast proliferation.

Medication use: Antibiotics, steroids, and chemotherapy all increase the risk of candidiasis. Antibiotics kill beneficial bacteria that normally keep Candida in check, while steroids suppress immune function.

Hormonal changes: Pregnancy increases susceptibility to both vaginal and oral candidiasis due to hormonal shifts and temporary immune changes.

Environmental factors: Warm, moist conditions favor Candida growth. This is why infections commonly occur in skin folds, in people who sweat heavily, or in those who wear tight, non-breathable clothing.

Medical devices: Central venous catheters and other invasive medical devices increase the risk for invasive candidiasis in hospitalized patients.

How candidiasis is diagnosed

Diagnosis typically begins with a physical examination and medical history. Healthcare providers look for characteristic signs such as white patches in the mouth or a red, itchy rash in skin folds.

For oral thrush, the appearance alone is often sufficient for diagnosis. However, if the infection doesn’t respond to treatment or if there’s concern about deeper infection, additional tests may be ordered.

For vaginal candidiasis, a vaginal culture or PCR test can identify the specific Candida species causing the infection. This is especially important for complicated or recurrent cases, as some species like Candida glabrata are intrinsically resistant to certain antifungal medications.

For invasive candidiasis, blood cultures are essential. Species identification and antifungal susceptibility testing help guide appropriate treatment choices.

Treatment approaches

Antifungal medications are the cornerstone of candidiasis treatment. The type, dose, and duration depend on the infection’s location and severity.

Oral thrush

Mild cases are treated with topical antifungals applied directly inside the mouth. Common medications include nystatin oral suspension, clotrimazole lozenges, and miconazole gel. If the infection doesn’t respond to topical treatment, oral antifungals like fluconazole may be prescribed. For immunocompromised patients with severe or esophageal involvement, systemic antifungal therapy is necessary.

Vaginal candidiasis

Uncomplicated vaginal yeast infections can often be treated with short-course topical antifungal creams, ointments, or suppositories (available over the counter) or a single dose of oral fluconazole. Complicated cases require longer therapy-typically 7 to 14 days of topical treatment or multiple doses of oral fluconazole. Women with recurrent infections may need maintenance antifungal regimens for six months.

Cutaneous candidiasis

Skin infections are typically managed with topical antifungal creams or powders such as clotrimazole, miconazole, or nystatin. Keeping the affected area dry is equally important. For nail infections, treatment may require oral antifungal medication taken for several weeks to months due to the slow growth rate of nails.

Invasive candidiasis

Bloodstream and organ infections require intravenous antifungal therapy. Echinocandins are typically the first-line treatment for most adult patients with candidemia. Treatment continues for at least two weeks after blood cultures become negative and symptoms resolve. Removal of any central venous catheters is also recommended when feasible.

Prevention strategies

Preventing candidiasis involves addressing modifiable risk factors:

  • Practice good hygiene: Keep skin clean and dry, especially in areas prone to moisture accumulation
  • Maintain good oral health: Brush twice daily, floss regularly, and rinse your mouth after using corticosteroid inhalers
  • Control blood sugar: For diabetic patients, well-managed glucose levels reduce the risk of yeast overgrowth
  • Use antibiotics judiciously: Avoid unnecessary antibiotic use, as this disrupts the body’s natural microbial balance
  • Wear breathable fabrics: Loose-fitting clothes and cotton underwear help prevent moisture buildup
  • Change wet or sweaty clothing promptly: This is especially important after exercising or swimming

For high-risk patients, such as those undergoing chemotherapy or with HIV/AIDS, preventive antifungal therapy may be considered.

Nursing considerations

As a nursing professional, you play a vital role in identifying candidiasis early and educating patients about prevention and treatment. Key points include assessing patients for risk factors, monitoring for signs of infection (particularly in immunocompromised patients), ensuring proper administration of antifungal medications, and teaching patients about completing their full course of treatment even after symptoms improve.

For hospitalized patients with invasive candidiasis risk, proper infection control practices-including appropriate catheter care and hand hygiene-are essential preventive measures.
