What are hepatitis viruses?
Hepatitis refers to inflammation of the liver, and while this condition can result from various factors like alcohol consumption or autoimmune disorders, viral hepatitis remains the most common cause globally. Scientists have identified five main hepatitis viruses, labeled A through E, each with distinct characteristics, transmission routes, and health impacts.
Beyond these five, researchers have also identified hepatitis G virus, now called human pegivirus. However, the evidence has not established it as an independent cause of hepatitis, though it often appears as a coinfection in people with chronic hepatitis B or C.
The different types of hepatitis viruses
Hepatitis A virus
Hepatitis A is an RNA virus from the Picornaviridae family that primarily spreads through the fecal-oral route. When someone ingests food or water contaminated with fecal matter containing the virus, they can become infected. This type of hepatitis is typically an acute, short-term illness. Most people recover completely and develop lifelong immunity without requiring specific treatment. In developed countries, international travel represents the most common risk factor for contracting this infection.
Hepatitis B virus
Hepatitis B stands out as the only DNA virus among the hepatitis viruses. This characteristic allows it to integrate into human DNA, taking up residence in liver cells. The WHO estimates that 254 million people were living with chronic hepatitis B infection in 2022, making it a major global health concern.
The virus transmits through contact with infected blood, semen, and other body fluids. Common transmission routes include mother-to-child transmission during birth, unsafe injections, contaminated blood transfusions, and sexual contact. Healthcare workers face occupational risks from accidental needlestick injuries. The timing of infection significantly impacts outcomes: while less than 5% of adults develop chronic infection, about 95% of infants and young children who contract the virus progress to chronic hepatitis B.
Hepatitis C virus
Hepatitis C is an RNA virus belonging to the Flaviviridae family. The virus primarily spreads through exposure to infected blood, particularly through sharing needles during drug injection, contaminated medical procedures, and blood transfusions. How chronic hepatitis leads to liver cancer

Many people with acute hepatitis experience mild symptoms or remain asymptomatic. When symptoms do appear, they typically include jaundice, dark urine, extreme fatigue, nausea, vomiting, and abdominal pain. The similarity of symptoms across different hepatitis viruses makes laboratory testing essential for accurate diagnosis.
For hepatitis B specifically, several blood tests help distinguish acute from chronic infection and assess liver damage severity. The hepatitis B surface antigen test indicates active infection, while antibody tests reveal past infection or vaccination status. Physical examination, ultrasound, and elastography can evaluate liver fibrosis and monitor disease progression.
Prevention and treatment strategies
Prevention remains the most effective approach to managing hepatitis viruses. Safe and effective vaccines exist for hepatitis A and B, with the hepatitis B vaccine providing protection for at least 20 years and likely for life. All infants should receive the hepatitis B vaccine within 24 hours of birth, followed by additional doses. The vaccine also protects against hepatitis D, since that virus cannot exist without hepatitis B.
For hepatitis C, no vaccine is currently available, making prevention through harm reduction strategies crucial. This includes safe injection practices, screening blood products, and proper sterilization of medical equipment.
Treatment options vary by virus type. Acute hepatitis A and E typically resolve without specific treatment, requiring only supportive care. Chronic hepatitis B can be managed with antiviral medications like tenofovir or entecavir, which slow cirrhosis progression, reduce liver cancer risk, and improve long-term survival. Most people who start hepatitis B treatment must continue it for life. Hepatitis C treatment has seen remarkable advances, with modern direct-acting antiviral drugs achieving cure rates around 90%.
The global burden and public health response
Viral hepatitis represents a major global health challenge. Approximately 304 million people worldwide live with chronic hepatitis B or C, and these infections cause an estimated 1.3 million deaths annually. The burden falls disproportionately on certain regions, with the WHO Western Pacific and African regions accounting for the highest numbers of chronic infections.
Despite the availability of effective vaccines and treatments, many people remain undiagnosed and untreated. As of 2022, only 13% of people with hepatitis B were aware of their infection, and just 3% were receiving treatment. This gap highlights the urgent need for expanded testing programs, improved access to care, and targeted interventions for high-risk populations including healthcare workers, people who inject drugs, and individuals in endemic regions.
