What are cutaneous mycoses?
Cutaneous mycoses are fungal infections affecting the superficial layers of skin, hair, and nails. These infections primarily target keratinized tissues, areas rich in the protein keratin that fungi can break down and use as a nutrient source. The condition is particularly prevalent in warm, humid climates where fungal growth thrives.
The fungi responsible for most cutaneous mycoses belong to a specialized group called dermatophytes. These organisms have evolved specifically to digest keratin, which explains why they colonize the outer layers of skin, hair, and nails rather than invading deeper tissues. Dermatophytes are restricted to keratinized tissues and rarely cause systemic infections, even in immunocompromised patients.
The three key dermatophyte genera
Dermatophyte infections are caused by three main fungal genera: Trichophyton, Epidermophyton, and Microsporum. Each genus has distinct characteristics and site preferences.
Trichophyton
Trichophyton is the most versatile genus, capable of infecting skin, hair, and nails. Species like Trichophyton rubrum and Trichophyton interdigitale are the most common culprits in dermatophyte infections worldwide. These fungi are responsible for the majority of athlete’s foot cases and nail infections.
Epidermophyton
Epidermophyton floccosum has a more limited range, infecting only skin and nails without affecting hair shafts and follicles. This species commonly causes infections in the groin area and feet.
Microsporum

Common types of dermatophyte infections
Dermatophyte infections are classified based on the body part they affect, using the term “tinea” followed by the Latin name for that location. Understanding these classifications helps in recognizing and treating these conditions.
Tinea pedis: athlete’s foot
Tinea pedis, commonly known as athlete’s foot, is one of the most prevalent fungal infections worldwide, with estimates suggesting that 70% of the population will experience it at some point in their lives. This infection typically affects the spaces between toes and the soles of the feet.
The condition thrives in warm, moist environments, making swimming pools, communal showers, and locker rooms common transmission sites. Symptoms include itching, scaling, cracked skin between the toes, and sometimes blisters. In severe cases, the infection can spread across the entire sole in a pattern called “moccasin distribution.”
Tinea corporis: ringworm of the body
Tinea corporis affects the skin on the body, typically presenting as circular, scaly patches with a distinctive ring-like appearance. These lesions often show central clearing with an inflamed, scaly border, which gives rise to the common name “ringworm” despite no actual worm being involved.
This infection can be transmitted through direct contact with infected individuals, animals, or contaminated objects. Children are particularly susceptible to zoophilic (animal-derived) infections from household pets.
Tinea capitis: scalp ringworm
Tinea capitis primarily affects children between 6 months and 12 years of age. This infection of the scalp and hair shafts can cause patchy hair loss, scaling, and inflammation. In severe cases, it may lead to a condition called kerion, which appears as a painful, pus-filled mass on the scalp.
Some Microsporum species causing tinea capitis emit a characteristic blue-green fluorescence under Wood’s lamp examination, which can aid in diagnosis.
Candida albicans: a different type of cutaneous infection
While dermatophytes cause most cutaneous mycoses, Candida species, particularly Candida albicans, are another important cause of cutaneous infections. Unlike dermatophytes, Candida is a yeast that naturally lives on the skin and mucous membranes as part of the body’s normal flora.
Cutaneous candidiasis occurs when there’s an overgrowth of Candida yeast due to an imbalance between healthy bacteria and yeast on the body. This infection typically affects skin folds such as underarms, under breasts, and the groin area where warmth and moisture create favorable conditions for yeast growth.
Key differences from dermatophyte infections
Candidal infections differ from dermatophytoses in appearance and predisposing factors. They typically present as bright red, moist patches with satellite lesions (small pustules or papules) beyond the main infected area. Common risk factors include diabetes, obesity, antibiotic use, immunosuppression, and conditions that create prolonged moisture on the skin.
Candida is the most common cause of diaper rash in infants, taking advantage of the warm, moist environment inside diapers.
Diagnosing cutaneous mycoses
Accurate diagnosis is essential for effective treatment. While clinical appearance often suggests the diagnosis, laboratory confirmation helps distinguish fungal infections from other skin conditions with similar presentations.
Healthcare providers typically use several diagnostic methods:
Clinical examination: Assessment of the lesion’s appearance, distribution, and patient history provides initial clues.
Wood’s lamp examination: This ultraviolet light test can detect certain fungi, particularly some Microsporum species, which fluoresce under UV light.
Microscopic examination: Skin scrapings, nail clippings, or hair samples are treated with potassium hydroxide (KOH) and examined under a microscope to identify fungal elements.
Fungal culture: Samples are cultured on specialized media to identify the specific fungal species, though results may take several weeks.
Treatment approaches
Treatment of cutaneous mycoses involves both medication and lifestyle modifications to create an environment unfavorable for fungal growth.
Topical antifungal medications
For localized, uncomplicated infections, topical antifungal agents are the first-line treatment. These medications include azoles (clotrimazole, miconazole), allylamines (terbinafine), and other agents. Treatment typically continues for 1-2 weeks after symptoms resolve to prevent recurrence.
Systemic antifungal medications
More extensive infections, nail involvement, or scalp infections often require oral antifungal medications. Common systemic treatments include terbinafine, itraconazole, and fluconazole. These medications require careful monitoring for potential side effects and drug interactions.
Supportive measures
Keeping affected areas clean and dry is crucial for successful treatment. This includes thorough drying after bathing, using moisture-wicking fabrics, and applying antifungal powders to maintain dryness in susceptible areas.
Prevention strategies
Preventing cutaneous mycoses involves creating conditions that discourage fungal growth:
Maintain good hygiene: Regular bathing with thorough drying, especially in skin folds and between toes, reduces fungal colonization.
Keep skin dry: Use moisture-wicking socks, breathable footwear, and change out of wet clothing promptly.
Avoid contaminated surfaces: Wear sandals in communal showers, swimming pools, and locker rooms to prevent contact with infected surfaces.
Manage underlying conditions: Control diabetes and other conditions that increase susceptibility to fungal infections.
Avoid sharing personal items: Don’t share towels, shoes, or clothing that may harbor fungal spores.
When to seek medical attention
While many cutaneous mycoses respond well to over-the-counter treatments, certain situations warrant professional medical evaluation. Seek care if the infection doesn’t improve after two weeks of home treatment, spreads to new areas, shows signs of bacterial infection (increased redness, warmth, pus), or if you have diabetes or a weakened immune system.
