With better use of primary prevention and health promotion strategies, research suggests the global burden of disease could be reduced by as much as 70%.
Primary prevention: stopping disabilities before they start
Primary prevention focuses on preventing the actual occurrence of disabilities by addressing risk factors before any damage occurs. These measures target populations at large or specific high-risk groups and include a wide range of interventions from preconception care to immunization programs.
Antenatal and natal care
Quality prenatal care is one of the most effective ways to prevent congenital disabilities. Regular prenatal visits allow healthcare providers to monitor fetal development, identify potential complications early, and provide essential guidance to expectant mothers. The Centers for Disease Control and Prevention (CDC) emphasizes that regular visits with healthcare providers before and during pregnancy are crucial for discussing medical conditions, family history of birth defects, and medication safety.
During pregnancy, several factors can influence fetal development. Healthcare providers counsel pregnant women about managing pre-existing conditions like diabetes, which has been linked to heart, spine, and brain defects in newborns. Women with diabetes can significantly reduce their baby’s risk of congenital heart defects by managing blood sugar levels before and during pregnancy.
Safe delivery practices during the natal period are equally important. Proper obstetric care helps prevent complications like birth asphyxia, intracranial hemorrhage, and trauma during labor-all of which can lead to permanent disabilities. Skilled birth attendants and well-equipped facilities play essential roles in ensuring safe deliveries.
Genetic counseling
Genetic counseling serves as a cornerstone of primary prevention, particularly for families with a history of hereditary conditions. This process involves providing information about genetic risk, the nature of genetic disorders, and available prevention methods. The WHO Regional Office identifies three essential aspects of effective genetic counseling: accurate diagnosis, risk estimation, and preventive measures that benefit those being counseled.
Couples planning to conceive can undergo carrier screening to determine their risk of passing on genetic conditions. When both partners carry genes for conditions like thalassemia or sickle cell disease, counselors can discuss reproductive options and monitoring strategies. This proactive approach has proven remarkably successful-programs combining screening, counseling, and prevention have achieved substantial reductions in births affected by serious genetic disorders.
Nutrition and folic acid supplementation
Adequate nutrition during pregnancy directly impacts fetal development. Folic acid supplementation stands out as one of the most evidence-based interventions for preventing neural tube defects affecting the brain and spine. The American College of Obstetricians and Gynecologists (ACOG) recommends taking 400 micrograms of folic acid daily for at least one month before pregnancy and continuing throughout pregnancy.
Research shows that adequate folic acid intake can reduce the chance of neural tube defects like spina bifida and anencephaly by as much as 70%. Avoiding harmful substances

Illicit drugs like cocaine, opioids, and marijuana can also cross the placenta and affect fetal development. Healthcare providers should screen for substance use and provide resources for cessation support.
Immunization programs
Vaccination programs have dramatically reduced disabilities caused by infectious diseases. Immunization against poliomyelitis has nearly eliminated paralytic polio worldwide. Rubella vaccination prevents congenital rubella syndrome, which can cause deafness, heart defects, and intellectual disability. The WHO’s community-based rehabilitation guidelines emphasize that immunization programs should be available for specific diseases and high-risk groups, including tetanus vaccination for pregnant women.

Prenatal screening and diagnosis
Modern prenatal screening enables detection of many conditions before birth, giving families and healthcare teams time to plan appropriate interventions. Ultrasound imaging can identify structural abnormalities like heart defects, neural tube defects, and skeletal problems. Genetic testing through amniocentesis or chorionic villus sampling can diagnose chromosomal conditions like Down syndrome.
When abnormalities are detected, specialized fetal therapy centers can sometimes perform interventions before birth. For conditions like twin-to-twin transfusion syndrome, laser surgery performed in utero has achieved survival rates of approximately 95% for at least one twin-outcomes that would be impossible without early detection.
Newborn screening programs
Newborn screening represents one of public health’s greatest success stories. The CDC describes newborn screening as identifying conditions that affect long-term health or survival, enabling early intervention that prevents death or disability and helps children reach their full potential.
Modern screening panels test for numerous conditions using a small blood sample collected from the newborn’s heel. These include metabolic disorders like phenylketonuria (PKU), endocrine conditions like congenital hypothyroidism, hemoglobin disorders including sickle cell disease, and hearing loss. For PKU, children who follow a restricted diet from infancy develop normally, whereas untreated children would develop severe intellectual disability.
Point-of-care screenings at birth also check for critical congenital heart disease using pulse oximetry and hearing loss using specialized testing. These screenings catch conditions that may not be immediately apparent but can cause serious developmental problems if left untreated.
Continuous assessment and early medical intervention
Beyond the newborn period, continuous developmental assessment helps identify disabilities early. Pediatric healthcare visits include developmental milestone checks that can flag delays in motor skills, language, social interaction, or cognitive abilities. When delays are identified, early intervention services can begin promptly.
Early intervention programs provide therapies and support services that can significantly improve outcomes for children with disabilities or developmental delays. Research shows that early intervention enhances parents’ capacity to care for and teach their children while improving family quality of life and reducing stress. Spacing pregnancies appropriately reduces risks associated with maternal exhaustion and nutritional depletion. Avoiding pregnancy at very young or advanced maternal ages decreases certain risks. Family planning counseling also provides opportunities to discuss preconception health, address chronic conditions, and ensure optimal health before conception.
Education extends beyond clinical settings into communities. Public health campaigns raise awareness about prevention strategies, encouraging behaviors that support healthy pregnancies and child development.
