Drug abuse remains a critical public health concern affecting individuals across all demographics. While many people use medications appropriately for medical purposes, drug abuse occurs when substances are used in ways that differ from their intended purpose, leading to serious physical, psychological, and social consequences. Understanding the nature of drug abuse, recognizing warning signs, and identifying commonly abused substances are essential steps in prevention and early intervention, particularly for healthcare professionals working in community settings.

What is drug abuse?
Drug abuse, also referred to as substance misuse, involves using a substance in a manner inconsistent with medical or legal guidelines. This can mean taking a medication in doses larger than prescribed, using someone else’s prescription, or using illegal drugs for their psychoactive effects. What distinguishes drug abuse from casual use is the compulsive pattern despite negative consequences, including harm to health, relationships, work, or school performance.
The distinction between drug use and abuse is important. Not everyone who uses a substance will develop a problem, but repeated use can quickly alter brain function, leading to addiction-a chronic disorder characterized by compulsive drug seeking despite harmful outcomes.
Physical and psychological dependence
Drug abuse often leads to two types of dependence that can occur separately or together.
Physical dependence
Physical dependence develops when the body adapts to regular drug exposure. When use stops, withdrawal symptoms emerge as the body readjusts to functioning without the substance. These symptoms vary by drug type but can include tremors, sweating, nausea, seizures, and in severe cases involving alcohol or barbiturates, withdrawal can be life-threatening. Physical dependence commonly occurs with alcohol, amphetamines, barbiturates, opiates, and nicotine.
Psychological dependence
Psychological dependence refers to the emotional and mental need for a drug. People with psychological dependence may use drugs to relieve psychological distress, seek pleasure, or cope with anxiety and depression. This form of dependence involves intense cravings and compulsive use patterns. Stimulants like cocaine produce strong psychological dependence, making it extremely difficult for users to stop even when they want to.
It’s important to note that these two types of dependence are not mutually exclusive-most substance use disorders involve both physical and psychological components working together to maintain addictive behavior.
Recognizing signs of drug abuse
Early identification of drug abuse can make a significant difference in outcomes. One key sign of addiction is continuing to use drugs despite harm to physical health, performance at school or work, or relationships.
Behavioral changes
Behavioral indicators are often the first noticeable signs. These include hanging out with different friends than usual, not caring about appearance, and losing interest in favorite activities. People struggling with substance abuse may become withdrawn, secretive, or display sudden mood swings. They might neglect responsibilities, miss work or school frequently, and engage in risky behaviors.
Physical signs
Physical manifestations vary depending on the substance used but can include bloodshot eyes, changes in appetite or sleep patterns, sudden weight loss or gain, poor hygiene, and unusual smells on breath or clothing. Observable evidence may include needle marks on arms or legs in opiate users, redness of eyes and rapid heartbeat in marijuana users, and excessive sweating with clenched teeth in club drug users.
Academic and performance decline
Getting lower grades, missing classes, or skipping school entirely are common signs of substance abuse in students. Drug use impedes learning and memory, which directly affects academic performance. Research shows teens who abuse drugs have lower grades, higher absence rates, and increased likelihood of dropping out.
Heavy substance use can lead to problems with working memory and attention due to changes in brain activity, which then decrease academic performance and school engagement. The cognitive impairments may persist even after the immediate drug effects wear off, making it difficult for students to retain information or focus during class.
Commonly abused drugs
Drugs of abuse fall into several categories based on their effects on the central nervous system.
Stimulants
Stimulants increase alertness, energy, and activity levels. They elevate blood pressure, heart rate, and respiration by enhancing the effects of neurotransmitters like dopamine and norepinephrine.
Amphetamines: These include prescription medications like Adderall and Dexedrine used for ADHD treatment, as well as methamphetamine. Users experience increased energy and feelings of pleasure, but repeated use can cause nausea, elevated blood pressure, anxiety, hallucinations, and paranoia. Long-term use leads to depletion of neurotransmitters, resulting in mood problems and cognitive issues.
Cocaine: Derived from coca plant leaves, cocaine produces intense but short-lived euphoria followed by feelings of power and confidence. Both cocaine and amphetamines act as dopamine agonists, which is why they have high abuse potential. Cocaine creates one of the strongest forms of psychological dependence among all drugs.
Depressants
Depressants slow down brain and body functions by enhancing GABA, a neurotransmitter that decreases neuronal excitability. Common depressants include alcohol, benzodiazepines, barbiturates, and inhalants.
Alcohol: The most widely used depressant, alcohol slows thinking and responses while lowering inhibitions. Alcohol abuse can cause memory loss, flushed skin, trembling hands, and with chronic use, serious health problems. Withdrawal from severe alcohol dependence can be life-threatening.
Barbiturates: Once commonly prescribed for anxiety and sleep, barbiturates carry high abuse potential. Chronic use at higher doses leads to physical dependence, and sudden withdrawal can be more severe than opiate withdrawal. Barbiturate addiction shows symptoms similar to chronic alcoholism including blackouts, slurred speech, poor coordination, and mood swings.
Benzodiazepines: Medications like Xanax, Valium, and Ativan treat anxiety and sleep disorders. These drugs can lead to both physical and psychological dependence, giving them relatively high abuse potential. Combining benzodiazepines with alcohol or opioids significantly increases risk of overdose.
Hallucinogens
Hallucinogens alter perceptions of reality by disrupting brain activity affecting mood, sensory perception, and muscle control. Users may see things that don’t exist or experience exaggerated sensory experiences like seeing colors more vividly.
Common hallucinogens include LSD, psilocybin mushrooms, and ketamine. While these drugs don’t typically produce physical addiction, prolonged use can result in deterioration of mental function. The effects are unpredictable and heavily influenced by the user’s mood and environment.
Marijuana (Cannabis)
Marijuana is unique because it acts like both a hallucinogen and a depressant. Studies show marijuana affects attention, memory, and learning ability, with effects lasting days or weeks after use. Heavy marijuana use during teenage years and into adulthood can reduce IQ by up to 8 points. Students who use marijuana regularly tend to get lower grades and have higher dropout rates.
The impact on brain function
Drug use causes changes in brain structure and function, leading to intense cravings, personality changes, and altered behaviors. These neurological changes affect areas responsible for judgment, decision-making, learning, and memory. Understanding that addiction is fundamentally a brain disorder helps explain why people cannot simply “choose” to stop using drugs-the changes in neural pathways make cessation extraordinarily difficult without proper support and treatment.
