Understanding the liver’s role in children
The liver is the body’s second largest organ, situated in the right side of the abdominal cavity. It performs numerous critical functions including converting nutrients from food into forms the body can use, cleaning harmful substances from the bloodstream, producing bile for digestion, and making proteins that allow blood to clot normally. In children, the liver also supports rapid growth and development, making any disruption to its function especially concerning.
The portal vein carries blood from the intestines and spleen to the liver, supplying approximately 75% of the liver’s blood and half of its oxygen. Unlike other organs that receive blood through arteries, the liver primarily relies on this venous blood flow. This unique arrangement allows blood to move slowly through the liver, giving liver cells time to process nutrients and filter toxins. When disease affects the liver or its blood vessels, this delicate system can become compromised.
Indian childhood cirrhosis: causes and characteristics
Indian childhood cirrhosis (ICC) is a chronic liver disease characterized by progressive scarring associated with copper deposition in the liver. This condition primarily affects children between 1 and 3 years of age and has historically been prevalent in the Indian subcontinent, particularly among middle-class families. While the exact cause remains under investigation, research suggests that both genetic susceptibility and environmental factors, particularly dietary copper exposure, play significant roles.
The copper connection
A striking observation in ICC cases is that affected children often consumed milk that was boiled and stored in copper or copper-alloy vessels. This practice can lead to leaching of copper from these vessels into the milk, resulting in excessive copper intake. The liver copper concentrations in affected children can reach extremely high levels, sometimes exceeding those seen in Wilson’s disease. However, unlike Wilson’s disease, ceruloplasmin levels in ICC are typically normal or elevated.
The condition has become less common in recent decades, largely due to increased awareness about avoiding copper cookware for infant feeding. Parents are now advised to use stainless steel or other non-reactive materials for preparing and storing infant formula and milk.
Clinical presentation
Children with Indian childhood cirrhosis typically present in the second year of life with abdominal distension and hepatosplenomegaly, often preceded by vague gastrointestinal symptoms and sometimes low-grade fever. Jaundice may be present early in the disease. As the condition progresses, children may show poor growth and weight gain, irritability, and increasing abdominal swelling.
The disease can present in two forms. The insidious onset involves gradual development of symptoms over weeks to months. The acute onset presents more dramatically with sudden fever, jaundice, and clay-colored stools, with rapid deterioration potentially leading to hepatic coma. Physical examination typically reveals a firm, enlarged liver with sharp borders.
Diagnosis and treatment
Diagnostic evaluation includes liver function tests showing elevated ALT and GGT, assessment of clotting times, and liver biopsy to examine the extent of scarring. Cupruresis testing, which measures copper excretion after D-penicillamine administration, helps confirm the diagnosis.
Treatment focuses on copper chelation therapy using D-penicillamine at doses of 20-40 mg/kg/day for 12-18 months. This medication helps remove excess copper from the body, and studies have shown it significantly reduces both serum and hepatic copper content while improving clinical symptoms. Supportive care includes adequate nutrition, treatment of infections with antibiotics, and symptomatic management. In cases of end-stage liver disease, liver transplantation may be necessary.
Portal hypertension in children
Portal hypertension refers to increased pressure within the portal vein system that carries blood from the digestive organs to the liver. While portal hypertension is often a complication of advanced liver disease, in children it can also result from blockages in the portal vein itself. This condition can lead to serious complications including gastrointestinal bleeding, enlarged spleen, and fluid accumulation in the abdomen.
Causes in pediatric patients
The causes of portal hypertension in children differ from those in adults. Prehepatic blockages affecting the portal vein before it reaches the liver are the most common cause in children, often resulting from blood clots or narrowing of the vessel. Cirrhosis is the second most common cause in pediatric patients, though it remains the primary cause in adults.
Blood clots in the portal vein can occur due to underlying clotting disorders, previous umbilical vein catheter placement in newborns, or sometimes for unknown reasons. When blockages occur, the body attempts to compensate by growing new veins to bypass the obstruction, creating what is called cavernous transformation of the portal vein. These bypass vessels are often twisted and fragile, contributing to increased pressure in the portal system.
Signs and symptoms
Portal hypertension itself often produces no symptoms initially. However, as the condition progresses, children may develop several concerning signs. Splenomegaly occurs because high pressure in the portal vein backs up into the splenic vein, causing the spleen to enlarge. An enlarged spleen removes more blood cells than normal, particularly platelets, leading to easy bruising and nosebleeds.
Ascites, or fluid accumulation in the abdomen, develops when high pressure forces fluid out of blood vessels into the abdominal cavity. Children may also develop visible veins under the skin of the abdomen as blood seeks alternative routes back to the heart.
The most serious complication involves varices, which are enlarged, fragile veins that form in the esophagus, stomach, or intestines. Upper gastrointestinal bleeding can be the first presenting symptom of pediatric portal hypertension. Signs of variceal bleeding include vomiting blood, black tarry stools, abdominal pain, dizziness, and in severe cases, collapse. Any suspicion of bleeding requires immediate emergency medical attention.
Diagnosis and management
Diagnosis typically involves physical examination, blood tests to assess liver function and platelet count, and imaging studies. Ultrasound can show the direction and speed of blood flow through the portal vein and help identify whether varices have developed. CT angiography or MRI may provide more detailed images of the blood vessels.
Endoscopy allows direct visualization of varices in the esophagus and stomach. This procedure is crucial both for diagnosis and for grading the severity of varices to determine appropriate treatment.
Treatment approaches include medications such as beta-blockers to help lower pressure within the portal vein. For varices, banding involves placing medical rubber bands over the affected blood vessels to cause them to clot and reduce bleeding risk. Sclerotherapy, where a chemical is injected into varices, provides an alternative when banding is not possible, particularly in very young children.
In cases where these interventions are insufficient, surgical procedures may be considered. The meso-rex bypass, or rex shunt, restores normal blood flow to an otherwise normal liver and is considered the treatment of choice for children with portal vein blockage. For children with advanced liver disease causing portal hypertension, liver transplantation may ultimately be necessary.
Nursing care essentials
Nursing care for children with liver disorders requires a comprehensive, multidisciplinary approach. Nutritional support is paramount, as malnutrition is common in pediatric liver disease and significantly affects both morbidity and mortality. Children may need increased calories, typically 120-150% of recommended intake, with careful attention to protein quality and fat-soluble vitamin supplementation.
Key nursing interventions include regular monitoring of abdominal girth to detect fluid accumulation, maintaining accurate intake and output records, and providing small, frequent meals to accommodate reduced appetite. Nurses should monitor for signs of bleeding, infection, and changes in mental status that might indicate hepatic encephalopathy.
Prevention and parent education
For Indian childhood cirrhosis specifically, prevention involves avoiding copper cookware for preparing and storing infant feeds. Parents should be advised to breastfeed exclusively for the first six months and use non-reactive containers when formula or milk supplements are introduced.
Education for families of children with portal hypertension includes recognizing signs of bleeding, understanding the importance of avoiding medications like aspirin and ibuprofen that increase bleeding risk, and knowing when to seek emergency care. While children with enlarged spleens should avoid contact sports, most physical activities should be encouraged to maintain overall health and development.
Infection prevention through aseptic techniques and proper hand hygiene is essential, as liver disease can impair immune function. Regular follow-up appointments, medication compliance, and dietary adherence are all critical components of long-term management.
