Burn injuries remain one of the most common and devastating forms of trauma affecting children worldwide. Unlike adults, children have thinner skin, larger body surface area relative to their weight, and unique physiological responses that make burns particularly dangerous. For nurses working in pediatric emergency settings, understanding burn assessment, immediate care, and ongoing management is critical to improving outcomes and minimizing long-term complications.

Understanding pediatric burns and their causes

Burns in children result from damage to the skin and underlying tissues caused by heat, electricity, chemicals, or radiation. Nearly a quarter of all burn injuries occur in children under 16 years of age, with the majority happening in those under five years old. The types of burns vary depending on the child’s age and developmental stage.

Scald injuries are the most common type of thermal injury in young children, accounting for nearly half of all pediatric burns. These typically occur when toddlers pull hot drinks or liquids off tables onto themselves. Flame or fire injuries become more prevalent in older children and adolescents. Contact burns from touching hot objects like irons, stoves, or curling irons are particularly common in infants and crawling babies. Electrical burns may occur when children chew on electrical cords or insert objects into outlets, while chemical burns can result from exposure to household cleaning products or industrial substances.

Classification of burns by depth and severity

Accurate burn classification is essential for determining treatment approaches and predicting outcomes. Burns are classified based on both depth of tissue damage and the total body surface area (TBSA) affected.

Burn depth classification

Superficial burns affect only the epidermis, appearing red with erythema but without blistering. These typically heal within 3-5 days without scarring. Superficial partial-thickness burns involve the epidermis and upper dermis, presenting with bright pink or red skin, blisters, moisture, and significant pain. Deep partial-thickness burns extend further into the dermis, appearing white to red with less blanching and reduced pain due to nerve damage. According to American Burn Association criteria, burn classification focuses on four components: appearance, blanching to pressure, pain, and sensation. Full-thickness burns destroy all layers of skin, appearing leathery or waxy with minimal pain. Fourth-degree burns extend beyond skin into fascia, muscle, or bone.

Severity categories

Based on extent and depth, pediatric burns are categorized as minor, moderate, or major. A severe burn in a pediatric patient is defined as greater than 10% TBSA affected, which contrasts sharply with adults where severe burns are defined as 20% or more TBSA. Burns involving the face, hands, feet, genitalia, or major joints are considered more significant regardless of size due to higher potential for functional impairment and complications.

Estimating burn area: Rule of Nines and modifications for children

Accurate estimation of TBSA is critical because it guides fluid resuscitation requirements and helps determine whether transfer to a specialized burn center is necessary.

The Rule of Nines

The Rule of Nines, also known as the Wallace Rule of Nines, divides the adult body into sections representing approximately 9% or multiples of 9% of TBSA. In adults, the head represents 9%, each arm is 9%, the anterior trunk is 18%, the posterior trunk is 18%, each leg is 18%, and the perineum is 1%.

Pediatric modifications

Children have proportionally larger heads and smaller legs compared to adults, requiring adjustments to the standard Rule of Nines. In children aged 1-4 years, the head accounts for approximately 19% of TBSA, while each leg represents a smaller percentage. For each year over one year old, 1% should be subtracted from the head and 0.5% added to each leg until adult proportions are reached around age 10-14.

The Lund-Browder chart provides more accurate TBSA estimation for children by accounting for age-specific body proportions. This chart assigns percentages to body segments that change with the child’s age. For smaller or scattered burns, the palmar method estimates the patient’s palm and fingers as representing approximately 1% of their body surface.

Immediate first aid and stopping the burning process

The first priority in pediatric burn management is stopping the burning process and preventing further injury. Remove the child from the heat source immediately and ensure scene safety. Remove clothing and jewelry from the affected area unless stuck to the burn.

Cool running water first aid

Current guidelines recommend applying cool running water for at least 20 minutes within three hours of the injury. Research involving nearly 2,500 children demonstrated that adequate first aid with cool running water significantly reduces skin grafting requirements and improves clinical outcomes. The Royal Children’s Hospital emphasizes that cooling reduces pain, swelling, and the risk of scarring-the sooner and longer the burn is cooled, the less impact the injury will have.

Important considerations for cool water first aid include using cool but not ice-cold water, focusing the water on the burn site rather than the whole body to prevent hypothermia, and avoiding ice, butter, toothpaste, or other home remedies that can worsen the injury. After cooling, cover the burn with a clean, loose, non-stick dressing or plastic cling film to protect it from contamination.

Nursing assessment and ongoing care

After initial stabilization, comprehensive nursing assessment follows the ABCDE approach: Airway, Breathing, Circulation, Disability, and Exposure. This systematic approach ensures no critical issues are missed.

Airway and breathing assessment

Burns occurring in enclosed spaces or involving the face require careful airway evaluation. Predictors of significant inhalation injury include stridor, wheezing, drooling, hoarseness, facial burns, and soot in the mouth. Airway edema can progress rapidly, making early assessment and potential intubation critical in severe cases.

Fluid resuscitation and hydration

Major burns cause significant fluid shifts due to capillary leak and tissue edema. Any pediatric burn greater than 10% TBSA requires intravenous fluid replacement. The Parkland formula calculates fluid requirements: 4 mL of Lactated Ringer’s solution ร— body weight in kg ร— %TBSA burned, with half given in the first 8 hours and the remainder over the next 16 hours. Nurses must monitor urine output carefully, aiming for 1-2 mL/kg/hour in children under 30 kg.

Pain management

Burn pain is often undertreated in children. Effective pain management requires distinguishing between background pain and procedural pain associated with dressing changes. Morphine remains the most widely used analgesic at burn centers, though multimodal approaches incorporating acetaminophen, ketamine, and non-pharmacological techniques such as distraction and music therapy improve overall pain control. Administering pain medication 30 minutes before dressing changes significantly improves the child’s comfort during procedures.

Wound care and infection prevention

Burns compromise the skin’s protective barrier, making infection prevention paramount. Nursing interventions include maintaining strict hand hygiene before and after wound contact, using sterile technique during dressing changes, applying appropriate topical antimicrobial agents as ordered, and monitoring for signs of infection such as fever, increased pain, purulent drainage, or bad odor. Silver-impregnated dressings are commonly used for their antimicrobial properties. Closed blisters should generally remain intact, while open blisters require cleaning and appropriate coverage.

Complications of pediatric burns

Understanding potential complications helps nurses anticipate problems and intervene early.

Burn shock

Inflammatory mediators released after burn injury cause systemic capillary leak and significant fluid shifts, potentially leading to hypovolemic shock. This typically develops over the first 24 hours, peaking around 6-8 hours post-injury. Signs include decreased urine output, tachycardia, hypotension, and altered mental status. Aggressive fluid resuscitation guided by careful monitoring is essential.

Respiratory complications

Inhalation injury significantly increases morbidity and mortality. Carbon monoxide has 250-fold greater affinity for hemoglobin than oxygen, impairing tissue oxygen delivery. Treatment requires administration of 100% oxygen. Cyanide poisoning from burning synthetic materials causes cellular hypoxia and requires specific antidote therapy when suspected.

Infection and sepsis

Loss of skin integrity combined with immune suppression following major burns creates high infection risk. Bacterial translocation across the injured gut is an independent cause of septic shock in burn patients. Common infection sources include the burn wound itself, intravenous catheter sites, urinary catheters, and ventilator-associated pneumonia. Regular surveillance and prompt treatment of infections are essential.

Metabolic and nutritional complications

Major burns trigger a hypermetabolic state with increased energy expenditure and protein catabolism. Children with burns greater than 40% TBSA require early nutritional support to prevent muscle wasting and support wound healing. Additionally, children have large surface area to mass ratios, putting them at risk for hypothermia during assessment and treatment.

Transfer criteria and specialized care

Certain pediatric burns require transfer to specialized burn centers for optimal outcomes. These include burns greater than 10% TBSA in children under 10 years, burns involving the face, hands, feet, genitalia, perineum, or major joints, circumferential burns, electrical or chemical burns, burns with associated inhalation injury, and burns in children with significant underlying medical conditions.
