Whooping cough, medically known as pertussis, earned its name from the distinctive high-pitched “whoop” sound children make as they gasp for air after severe coughing episodes. As a pediatric nurse, understanding the comprehensive management of this highly contagious respiratory infection is essential-not just for treating the child, but for protecting entire families and communities from this potentially life-threatening disease.

The catarrhal stage lasts one to two weeks and resembles a common cold with runny nose, sneezing, low-grade fever, and mild cough. This is when patients are most contagious, yet diagnosis is often missed because symptoms appear so ordinary. During the paroxysmal stage, which can last one to six weeks, the characteristic whooping cough emerges. Children experience rapid, repeated coughing fits followed by the distinctive inspiratory whoop. These episodes can be so severe that children may vomit, turn blue from lack of oxygen, or become completely exhausted. The convalescent stage involves gradual recovery over weeks to months, though coughing episodes may recur with subsequent respiratory infections.
Comprehensive nursing assessment
Thorough assessment forms the foundation of effective pertussis management. Nurses must vigilantly monitor respiratory status, recognizing that infants present the highest risk for complications and may not display the classic whoop at all.
Respiratory monitoring priorities
Continuous pulse oximetry is essential, particularly during and immediately after coughing paroxysms when oxygen saturation can drop dramatically. Document the frequency, duration, and severity of coughing episodes. Watch for signs of respiratory distress including tachypnea, nasal flaring, intercostal retractions, and use of accessory muscles. Research published in StatPearls indicates that hospitalization is indicated for patients with hypoxia, pneumonia, central nervous system complications, or inability to tolerate oral nutrition and hydration.
In infants, be especially alert for apneic episodes, which may occur without any coughing. Apnea in young infants often presents with cyanosis and bradycardia, sometimes accompanied by brief loss of consciousness. These episodes require immediate intervention and close monitoring.
Hydration and nutritional assessment
The violent coughing episodes frequently trigger vomiting, making adequate nutrition and hydration challenging. Assess for signs of dehydration including decreased urine output, dry mucous membranes, and poor skin turgor. Monitor weight daily in hospitalized patients. Document feeding tolerance and any post-tussive vomiting that could compromise nutritional status.

Nursing care for pediatric pertussis patients combines supportive measures with pharmacological treatment, all while implementing strict infection control protocols.
Airway management and respiratory support
The CDC recommends that treatment is largely supportive, including oxygen therapy, suctioning, hydration, and avoiding respiratory irritants. Position patients in high Fowler’s position during coughing episodes to facilitate chest expansion and ease breathing. Gentle suctioning may be necessary to clear thick secretions, but aggressive suctioning should be avoided as it can trigger additional paroxysms.
Humidified oxygen should be administered as needed to maintain adequate oxygen saturation. Create a calm, quiet environment since stress, crying, and external stimuli can precipitate coughing fits. Keep the room at a comfortable temperature and minimize unnecessary procedures during recovery periods between paroxysms.
Antibiotic administration
Antibiotics are a critical component of pertussis treatment, though their effectiveness depends heavily on timing. According to CDC guidelines, treatment within the first one to two weeks before coughing paroxysms occur can lessen symptom severity. However, antibiotics given later in the illness will not alter its course-they serve primarily to reduce the period of communicability.
The recommended antibiotics include macrolides such as azithromycin, clarithromycin, and erythromycin. For infants less than one month of age, azithromycin is preferred despite a small association with infantile hypertrophic pyloric stenosis, because the risk of severe pertussis complications outweighs this potential side effect. Nurses must monitor young infants receiving macrolides for signs of pyloric stenosis, including projectile vomiting and failure to gain weight.
Trimethoprim-sulfamethoxazole serves as an alternative for patients two months and older, particularly when macrolide resistance is suspected. Ensure parents understand the importance of completing the entire course of antibiotics, even after symptoms improve, to prevent relapse and reduce transmission.
Infection control measures
Pertussis spreads through respiratory droplets during coughing and sneezing, making strict infection control essential. Place hospitalized patients on droplet precautions with a private room when possible. Healthcare workers should wear surgical masks when within three feet of the patient. Patients remain infectious from the catarrhal stage through the third week after paroxysms begin, or until five days after starting effective antibiotic therapy.
Special considerations for high-risk populations
While pertussis affects all age groups, certain populations require heightened attention and modified care approaches.
Infants under six months
The National Foundation for Infectious Diseases reports that whooping cough is most dangerous for infants younger than three months, and many are infected by older siblings, parents, or caregivers who may not realize they have the disease. Infants in this age group have the highest rates of hospitalization and mortality. They may present atypically, with apnea as the primary symptom rather than the classic whooping cough.
Monitor these youngest patients closely for complications including pneumonia, seizures, encephalopathy, and pulmonary hypertension. Even seemingly mild cases may warrant hospitalization due to the potential for rapid deterioration. The infant mortality rate from pertussis is approximately two percent of cases, with this age group accounting for the vast majority of pertussis-related deaths.
Children with underlying conditions
Patients with pre-existing respiratory conditions, immunodeficiency, or chronic illnesses require intensified monitoring. These children may experience more severe disease courses and higher complication rates. Coordinate closely with specialists and prepare for potential intensive care admission if respiratory status deteriorates.
Parent and family education
Effective discharge teaching significantly impacts outcomes and prevents disease spread within families and communities.
Home care instructions
Teach parents to recognize warning signs requiring immediate medical attention, including difficulty breathing, bluish discoloration of the lips or face, prolonged periods without breathing, inability to keep fluids down, or signs of dehydration. Explain that symptoms can persist for weeks to months-pertussis is sometimes called the “hundred-day cough”-so parents should be prepared for a prolonged recovery period.
Advise parents to keep the child’s environment calm and avoid triggers that may precipitate coughing episodes. Small, frequent meals may be better tolerated than larger portions. Ensure adequate rest while maintaining gentle activity as tolerated during the convalescent phase.
Prophylactic treatment for contacts
The CDC supports providing postexposure prophylaxis (PEP) to all asymptomatic household contacts of a pertussis case within twenty-one days of cough onset in the infected patient. This is crucial because secondary attack rates among susceptible household contacts can reach eighty percent. The antibiotic regimens for prophylaxis are identical to those used for treatment.
Special emphasis should be placed on protecting high-risk household members, including infants under twelve months, pregnant women in their third trimester, and individuals with pre-existing health conditions that could be worsened by pertussis infection. All close contacts should receive prophylaxis regardless of their vaccination status.
Prevention through vaccination
Vaccination remains the most effective strategy for preventing pertussis. The DTaP vaccine is recommended for infants and children, given at two, four, six, and fifteen to eighteen months of age, with a booster at four to six years. Adolescents and adults should receive Tdap boosters, as immunity from childhood vaccination wanes over time.
Particularly important is maternal vaccination during each pregnancy, ideally between twenty-seven and thirty-six weeks of gestation. This allows antibodies to transfer to the infant, providing protection during the vulnerable first months of life before the baby can receive their own vaccinations. Nurses should encourage the “cocooning” strategy, where all family members and caregivers of newborns are vaccinated to create a protective barrier around vulnerable infants.
Documentation and interdisciplinary collaboration
