When an infant struggles to breathe, every parent’s heart skips a beat. Bronchiolitis, an acute inflammation of the smallest airways in the lungs, is one of the most common respiratory infections affecting babies-particularly those under six months of age. This condition fills hospital wards during winter months and presents unique challenges for healthcare providers and families alike. Understanding its causes, recognizing symptoms early, and providing appropriate nursing care can make a significant difference in outcomes for these tiny patients.

What is bronchiolitis?
Bronchiolitis is an acute viral infection affecting the small breathing tubes of the lungs called bronchioles. When these airways become inflamed and filled with mucus, they swell and obstruct airflow, making breathing difficult for infants. Unlike bronchitis, which affects larger, central airways in adults, bronchiolitis specifically targets the smallest airways-a critical distinction that explains why infants are most severely affected.
The condition primarily impacts children under two years of age, with peak incidence occurring between 2 and 6 months. Why are infants so vulnerable? Their airways are significantly smaller than those of older children and adults, meaning even modest swelling can cause substantial obstruction. Additionally, infants rely heavily on nasal breathing and have difficulty clearing secretions from their airways.
The pathophysiology explained
When a virus invades the respiratory tract, it infects the epithelial cells lining the bronchioles. This triggers an inflammatory response that causes edema, increased mucus production, and the recruitment of inflammatory cells. As infected cells die, they slough off into the already congested airways, creating debris that further blocks airflow.
The resulting obstruction works like a ball-valve mechanism-air can enter the lungs during inhalation but becomes trapped during exhalation. This leads to hyperinflation of the alveoli, decreased lung compliance, and impaired gas exchange. In severe cases, areas of the lung may collapse completely, causing hypoxemia and respiratory distress.
What causes bronchiolitis?
Multiple viruses can cause bronchiolitis, but respiratory syncytial virus (RSV) is the leading culprit and the most common cause of infant hospitalization in the United States. RSV accounts for the majority of bronchiolitis cases, though in approximately 30% of affected infants, two viruses may be present simultaneously.
Other viral agents
Beyond RSV, several other pathogens can trigger bronchiolitis. These include human metapneumovirus, parainfluenza virus, adenovirus, rhinovirus, influenza, and coronaviruses including SARS-CoV-2. Occasionally, Mycoplasma pneumoniae has been associated with a bronchiolitis-like illness characterized by wheezing, though this is less common.
How transmission occurs
These viruses spread through contact with respiratory droplets when an infected person coughs or sneezes. Transmission can also occur when a child touches a contaminated surface-like a doorknob or toy-and then touches their eyes, nose, or mouth. RSV can survive on surfaces for several hours, making infection control measures particularly important in childcare settings and hospitals.
Risk factors for severe disease
While any infant can develop bronchiolitis, certain factors increase the risk of severe illness requiring hospitalization. Age remains the most significant predictor-infants six months or younger and premature babies are at greatest risk.
Other high-risk groups include infants with congenital heart disease, chronic lung disease, neuromuscular disorders affecting their ability to clear secretions, weakened immune systems, and those with airway abnormalities. Environmental factors also play a role-exposure to secondhand smoke compounds the severity of bronchiolitis in infants.
Clinical presentation and symptoms
Bronchiolitis typically begins with mild upper respiratory symptoms resembling a common cold. Parents may notice a runny nose, mild cough, and low-grade fever for the first one to two days. The illness then progresses as the infection moves into the lower respiratory tract.
Early warning signs
Within 48 to 72 hours of initial symptoms, lower airway involvement becomes evident. The increasing cough is usually the first sign of lower respiratory tract involvement, followed by faster breathing, wheezing, and increased effort to breathe. Parents may describe their baby as “breathing funny” or notice their infant working harder than usual during respiration.
Signs of severe disease
More concerning symptoms include nasal flaring, grunting sounds during breathing, and visible retractions-where the skin pulls in between the ribs, below the ribcage, or at the base of the throat with each breath. The infant may become irritable, lethargic, and refuse to feed. In severe cases, cyanosis develops, appearing as a bluish discoloration of the lips and fingertips.
Young infants under two months of age may present with apnea-brief episodes where they stop breathing altogether-before other typical symptoms appear. This makes close observation particularly crucial in the youngest patients.
Diagnosis and evaluation
Bronchiolitis is primarily a clinical diagnosis based on history and physical examination findings. Healthcare providers listen for characteristic wheezing, crackles, and rhonchi during auscultation. They assess respiratory rate, oxygen saturation, and hydration status.
Laboratory tests and imaging are generally unnecessary for uncomplicated cases. Viral testing may be considered for hospitalized infants, primarily for isolation and infection control purposes. A chest X-ray is only indicated when complications like pneumonia or pneumothorax are suspected.
Medical management approaches
The cornerstone of bronchiolitis treatment is supportive care. Unlike bacterial infections, there is no antiviral medication that effectively treats most causes of bronchiolitis. Management focuses on maintaining adequate hydration and oxygenation while the body fights off the viral infection.
Hydration support
Ensuring adequate fluid intake is critical, especially since infants with respiratory distress often struggle to feed. Mild cases can be managed with frequent, smaller feeds. More severe cases may require intravenous or nasogastric fluid administration when oral intake becomes impossible.
Oxygen therapy
Supplemental oxygen is provided when oxygen saturation drops below acceptable levels. Current guidelines suggest maintaining saturations at or above 90% in previously healthy infants. Oxygen can be delivered via nasal cannula, face mask, or oxygen hood depending on the severity and the infant’s tolerance.
What about medications?
Research has consistently shown that bronchodilators, corticosteroids, and antibiotics provide no significant benefit in bronchiolitis. The American Academy of Pediatrics clinical practice guidelines recommend against routine use of these medications. Nebulized hypertonic saline may help improve mucus clearance in hospitalized infants, though evidence remains mixed.
Essential nursing care interventions
Nurses play a pivotal role in managing infants with bronchiolitis. Their assessments, interventions, and education efforts directly impact patient outcomes and family coping.
Airway management
Maintaining a patent airway is the top nursing priority. This involves gentle nasal suctioning to clear nasal passages and positioning the infant to optimize breathing. Elevating the head of the bed and placing the infant in a slightly upright position can help reduce respiratory effort. Some evidence supports the prone position for improving diaphragmatic function in monitored hospital settings.
Respiratory monitoring
Continuous assessment of respiratory status is essential. Nurses monitor respiratory rate, work of breathing, oxygen saturation, and breath sounds. They watch for signs of deterioration including increasing tachypnea, worsening retractions, apneic episodes, and declining oxygen levels. Documentation should capture changes in respiratory effort and the infant’s response to interventions.
Fluid balance and nutrition
Nurses carefully monitor intake and output, including weighing all diapers to measure urine output. They assess for signs of dehydration such as decreased wet diapers, sunken fontanelle, and poor skin turgor. For infants requiring tube feeding, small frequent bolus feeds or continuous feeding at reduced volumes may be better tolerated.
Temperature management
While fever is common in bronchiolitis, managing elevated temperatures helps reduce metabolic demands and improves infant comfort. Antipyretics like acetaminophen can be administered as needed. The environment should be kept comfortable, and the infant should not be overdressed.
Family education and support
Parents often feel anxious and helpless when their infant is hospitalized. Nurses provide education about the disease process, expected timeline for recovery, and what signs indicate improvement or deterioration. Teaching proper nasal suctioning technique empowers parents to participate in care.
Key teaching points include the importance of handwashing to prevent spread, maintaining a smoke-free environment, encouraging small frequent feeds when the infant is able, and knowing when to seek emergency care. Most infants improve within 14 to 21 days as long as they remain well hydrated.
Prevention strategies
Prevention remains the best approach to reducing the burden of bronchiolitis. RSV immunization options now exist for protecting high-risk infants. Maternal RSV vaccination during pregnancy or infant immunization with monoclonal antibodies like nirsevimab can provide protection during the most vulnerable months of life.
General infection control measures include frequent handwashing, avoiding contact with sick individuals, keeping infants away from crowded settings during RSV season, and ensuring household members stay current with vaccinations. Breastfeeding also offers protective benefits against respiratory infections.
Prognosis and outcomes
The good news is that most infants with bronchiolitis recover completely with supportive care. Approximately 1% to 3% of affected children require hospitalization, and among hospitalized infants, most improve within a few days. Respiratory failure requiring mechanical ventilation is rare and typically affects those with underlying health conditions.
When recognized early and managed appropriately, the prognosis for bronchiolitis is excellent. Effective nursing care-focused on airway maintenance, hydration support, and vigilant monitoring-forms the foundation of successful treatment for these vulnerable young patients.
