Mumps remains a significant infectious disease in paediatric care, despite widespread vaccination efforts. This contagious viral illness primarily targets the salivary glands and can cause considerable discomfort in children. For paediatric nurses, understanding the disease process and implementing effective nursing strategies is essential to ensure optimal patient outcomes and prevent transmission to others.

Understanding mumps and how it spreads

Mumps is caused by a paramyxovirus, a single-stranded RNA virus that spreads through respiratory droplets when an infected person coughs, sneezes, or talks. The virus can also spread through direct contact with saliva or items contaminated with an infected person’s saliva, such as drinking glasses, utensils, or toys. Children are most contagious from two days before symptoms appear until five days after parotid swelling begins, making early identification crucial for infection control.

The incubation period typically ranges from 12 to 25 days, with most children developing symptoms 16 to 18 days after exposure. It’s worth noting that up to one-third of infected individuals may have very mild symptoms or remain asymptomatic, yet still capable of spreading the virus to others.

Recognising clinical signs and symptoms

Children with mumps typically present with a distinctive clinical picture that nurses must recognise promptly. The illness often begins with non-specific prodromal symptoms that appear several days before the characteristic glandular swelling.

Early prodromal symptoms

Initial symptoms include low-grade fever lasting three to four days, headache, muscle aches, fatigue, loss of appetite, and general malaise. These symptoms can easily be mistaken for other viral illnesses, so nurses should maintain a high index of suspicion, particularly during known outbreaks or when the child has had contact with confirmed cases.

Parotitis and glandular involvement

The hallmark of mumps is painful swelling of the parotid salivary glands, located between the ears and jaw. This swelling gives children the characteristic appearance of puffy cheeks and a swollen jaw. The swelling typically peaks within one to three days and subsides over the following week. In about 25% of cases, only one side is affected, though bilateral involvement is more common. Children often experience increased pain when chewing, swallowing, or consuming acidic foods and drinks like orange juice or citrus fruits.

Comprehensive nursing assessment

A thorough nursing assessment forms the foundation of effective mumps management. Nurses should systematically evaluate several key areas to guide care planning.

Physical assessment priorities

Begin by assessing vital signs, paying particular attention to temperature patterns. Examine the parotid and submandibular glands for swelling, tenderness, and warmth. Inspect the oral cavity, noting any redness or swelling around Stensen’s duct openings. Document the child’s ability to eat, drink, and swallow comfortably. Assess hydration status by checking skin turgor, mucous membranes, and urine output.

Pain assessment

Use age-appropriate pain scales to evaluate the child’s discomfort level. The pain associated with parotid swelling can be significant and typically worsens with jaw movement or when eating. Regular pain assessment allows nurses to evaluate the effectiveness of interventions and adjust care accordingly.

Symptom management strategies

Mumps is a self-limiting illness with no specific antiviral treatment. Nursing care therefore focuses on supportive measures to alleviate symptoms and promote comfort during recovery.

Pain relief interventions

Administer prescribed analgesics such as paracetamol (acetaminophen) or ibuprofen to reduce fever and manage pain. Never give aspirin to children with viral infections due to the risk of Reye’s syndrome, a potentially fatal condition affecting the liver and brain. Apply warm or cold compresses to the swollen parotid area, as many children find this soothing. Some children prefer warmth, while others respond better to cold packs, so individualise this intervention based on the child’s preference.

Hydration and nutritional support

Encouraging adequate fluid intake is essential, as children may resist drinking due to swallowing discomfort. Offer small, frequent sips of water, diluted fruit juice (avoiding citrus), milk, or ice chips. Soft, bland foods that require minimal chewing are best tolerated. Avoid acidic foods and beverages, vinegar-containing condiments, and foods with tomato base, as these stimulate saliva production and can intensify pain. Soups, mashed vegetables, yoghurt, and soft pasta are usually well-accepted options.

Comfort measures

Provide gentle oral care to maintain mouth hygiene, offering sips of water or ice chips to keep the mouth moist. Encourage rest to support the child’s recovery. Keep the child entertained with quiet activities like reading, puzzles, or watching films to distract from discomfort.

Monitoring for complications

While most children recover completely from mumps within two weeks, nurses must remain vigilant for potential complications that require prompt medical intervention.

Orchitis in males

Orchitis occurs in approximately 30% of unvaccinated post-pubertal males and about 6% of vaccinated males. Symptoms include testicular pain, swelling, and tenderness, usually affecting one testicle. Nursing management includes scrotal support, bed rest, and application of cold compresses. Although orchitis can cause testicular atrophy, complete infertility is rare. Educate parents about warning signs and the importance of seeking medical attention if testicular symptoms develop.

Meningitis and neurological complications

Mumps can cause viral meningitis, presenting with severe headache, neck stiffness, photophobia, and altered consciousness. Neurological complications are typically self-resolving, but children showing signs of central nervous system involvement require immediate medical evaluation. Encephalitis, though rare, can lead to seizures, changes in consciousness, and other serious consequences.

Other complications to monitor

Additional complications include pancreatitis (presenting with abdominal pain, nausea, and vomiting), oophoritis in post-pubertal females, and hearing loss. Instruct parents to watch for persistent high fever, severe headache, abdominal pain, or any concerning changes in their child’s condition.

Infection control and isolation measures

Preventing transmission is a critical nursing responsibility. The CDC recommends isolation for five days after the onset of parotid swelling to minimise the risk of spreading the virus to others.

Hospital and home isolation

In healthcare settings, implement standard and droplet precautions for all suspected or confirmed mumps cases. Place children in private rooms when possible. Ensure healthcare personnel caring for these patients have documented immunity to mumps. At home, advise parents to keep the child away from school, childcare facilities, and public places until the isolation period is complete. Siblings and other household members who are not immune should be monitored for symptoms.

Hand hygiene education

Teach thorough handwashing techniques to the child and family members. Explain the importance of washing hands frequently, especially after touching the face, before eating, and after using the bathroom. Discourage sharing of drinking glasses, utensils, and personal items. Instruct children to cover their mouth and nose when coughing or sneezing, preferably with a tissue or their elbow.

The importance of MMR vaccination

Vaccination remains the most effective strategy for preventing mumps. The measles-mumps-rubella (MMR) vaccine is administered in two doses, with the first dose given between 12 and 15 months of age and the second dose between 4 and 6 years.

Two doses of MMR vaccine provide approximately 88% protection against mumps. While vaccinated individuals can still contract mumps, they typically experience milder symptoms and fewer complications compared to unvaccinated individuals. Nurses play a vital role in promoting vaccination by educating families about the safety and effectiveness of the MMR vaccine.

Addressing vaccine hesitancy

When encountering vaccine-hesitant parents, provide evidence-based information about the benefits of vaccination and address concerns compassionately. Explain that adverse reactions to the MMR vaccine are rare and usually mild, such as temporary soreness at the injection site or low-grade fever. Document discussions about vaccination and provide resources for families to make informed decisions.

Family education and discharge planning

Comprehensive family education ensures continuity of care after discharge. Cover the following key points with parents and caregivers:

Disease course: Explain that parotid swelling typically lasts five to seven days and most children recover completely within two weeks. Warning signs: Instruct parents to seek immediate medical attention if the child develops severe headache, stiff neck, testicular pain, severe abdominal pain, persistent vomiting, or high fever. Return to activities: Children can return to school once symptoms have improved and it has been at least five days since the swelling started.
