A persistent wet cough in children should never be ignored, especially when it lingers for weeks. While many respiratory issues resolve with basic treatment, some conditions like bronchiectasis require more specialized care. This chronic lung disease affects the airways, causing them to become permanently widened and damaged. Early recognition and proper management are critical-particularly in young patients where timely intervention can sometimes reverse the damage.

What is bronchiectasis?

Bronchiectasis is a condition where damage causes the tubes in the lungs (airways) to widen or develop pouches. This abnormal dilation makes it difficult to clear mucus properly. The widened airways collect secretions that contribute to chronic cough, and bacteria can get trapped in this mucus, leading to recurring infections.

The condition follows a destructive cycle: airway injury leads to inflammation, which causes mucus accumulation and infection, which in turn causes more injury. When this cycle worsens, children experience increased coughing, known as an exacerbation. Unlike some respiratory conditions that come and go, bronchiectasis causes permanent structural changes to the bronchial walls, though the progression can be halted with appropriate care.

Causes of bronchiectasis in children

Understanding what triggers bronchiectasis helps guide treatment decisions. The damage to airways typically results from one or more underlying factors.

Respiratory infections are the most common cause of bronchiectasis in children. Most often, the damage comes from an infection, though the reason is unknown in about four out of every ten people with bronchiectasis. Severe pneumonia, measles, pertussis (whooping cough), and tuberculosis can all cause sufficient damage to initiate the disease process.

Protracted bacterial bronchitis-a long-lasting bacterial infection of the airways-has been recognized in recent years as a common cause of bronchiectasis. When this condition goes untreated, the persistent inflammation leads to permanent airway widening.

Several inherited conditions predispose children to bronchiectasis. In primary ciliary dyskinesia, the tiny mucus-clearing hairs (cilia) in the lungs don’t work properly, leading to mucus buildup. Cystic fibrosis causes thick, sticky mucus that’s difficult to clear, creating ideal conditions for infection and airway damage. Children with immune deficiencies face increased risk because they cannot fight off respiratory infections effectively.

Other contributing factors

Aspiration-when food, liquid, or stomach acid enters the lungs-can cause repeated injury leading to bronchiectasis. Airway obstruction from inhaled foreign objects, severe asthma, and autoimmune conditions that cause abnormal inflammation can also contribute. In some children, multiple factors work together to cause the condition.

Signs and symptoms to recognize

The hallmark symptom of bronchiectasis is a chronic wet or productive cough. This wet-sounding cough is often the main symptom and usually lasts for weeks, with extra mucus in the airways causing the cough. Children may spit up or cough up this mucus, which can be yellow or green in color and sometimes foul-smelling-indicating infection.

Additional symptoms include:

  • Increased mucus production that may worsen during exacerbations
  • Shortness of breath and difficulty with exercise
  • Chest pain during breathing or coughing
  • Poor growth or weight loss due to the energy demands of fighting infection
  • Hemoptysis (coughing up blood) in more severe cases
  • Clubbing-changes in the fingernails that can develop over time

A chronic wet cough is often the only symptom present for some children with bronchiectasis, making it easy to dismiss as something less serious. Healthcare providers may also note crackles (rattling sounds) when listening to the lungs with a stethoscope.

Diagnostic approaches

Accurate diagnosis is essential because bronchiectasis symptoms can be mistaken for asthma, chronic bronchitis, or viral infections. Several diagnostic tools help confirm the condition.

Imaging studies

A chest CT scan is the best way to evaluate the small airways and determine if they have been damaged. High-resolution computed tomography (HRCT) provides detailed images showing the characteristic airway dilation. While standard chest X-rays may detect severe cases, they often miss subtle bronchiectasis.

Bronchoscopy and laboratory tests

Bronchoscopy is done to look inside the child’s airway and lungs using a bronchoscope-a tube with a light on the end. This procedure allows doctors to collect samples for testing and may help identify the underlying cause. Pulmonary function tests measure how well the lungs use oxygen and help determine disease severity.

Additional tests may include sputum cultures to identify bacteria, sweat tests to rule out cystic fibrosis, and blood tests to assess immune function.

Medical management strategies

Treatment aims to break the cycle of infection, inflammation, and mucus accumulation. A comprehensive approach typically involves multiple interventions.

Antibiotic therapy

Treating acute respiratory exacerbations with antibiotics is considered standard care in children and adults with bronchiectasis. During flare-ups, oral antibiotics are commonly prescribed for two to eight weeks. Some children require long-term prophylactic antibiotics to prevent recurrent infections. Inhaled antibiotics may be necessary for children who grow certain bacteria like Pseudomonas aeruginosa in their mucus.

Mucolytics and bronchodilators

Management involves taking mucolytics such as hypertonic saline to thin out the mucus. These medications make secretions easier to cough up. Bronchodilators help relax and open the airways, improving airflow and making breathing easier during airway clearance sessions.

Surgical intervention

When bronchiectasis is localized to one area and doesn’t respond to other treatments, surgical removal of the affected lung segment may be considered. Surgery may be used to remove a part of the child’s lung causing symptoms, and in very severe cases, lung transplantation might be necessary.

Airway clearance techniques

Clearing mucus from the lungs is the cornerstone of bronchiectasis management. Airway clearance is the most important treatment for bronchiectasis-it helps move mucus from smaller airways to larger airways where it can be cleared more easily.

Postural drainage

Postural drainage involves positioning the patient with an involved lung segment such that gravity has maximum effect on facilitating drainage of secretions. Different positions target different lung segments-children may need to lie on their sides, back, or stomach, sometimes with the head lower than the chest. Sessions typically last 20-30 minutes and should be performed at least once or twice daily.

Percussion and vibration

The percussion technique entails gently striking or clapping the chest wall with cupped hands or a percussion device to help loosen and mobilize mucus trapped within the airways. Vibration involves applying rhythmic pressure to further dislodge secretions. These manual techniques are often combined with postural drainage.

Modern airway clearance devices

Several devices can assist with airway clearance. Positive expiratory pressure (PEP) devices, oscillating PEP devices (like Flutter valves or Aerobika), and high-frequency chest wall oscillation vests provide alternatives to manual techniques. The type of airway clearance recommended depends on factors including the cause of bronchiectasis, the child’s age, and development.

Nursing care priorities

Nurses play a vital role in managing children with bronchiectasis across hospital and home settings. Key nursing responsibilities include comprehensive assessment, medication administration, airway management, and family education.

Assessment and monitoring

Documentation should include respiratory rate, character of breath sounds, presence of cyanosis, frequency and appearance of secretions, and character of cough. Nurses monitor vital signs, oxygen saturation, and signs of respiratory distress. Tracking the frequency and severity of exacerbations helps evaluate treatment effectiveness.

Medication administration

Nurses ensure antibiotics, bronchodilators, and mucolytics are administered correctly and on schedule. For nebulized medications, proper technique and equipment cleaning are essential. Nurses also monitor for medication side effects and therapeutic response.

Facilitating airway clearance

Performing or supervising chest physiotherapy is a critical nursing intervention. Postural drainage should be performed with percussion and vibration in the morning and at night as prescribed. Nurses should encourage effective coughing techniques (huff coughs) and ensure treatments are timed appropriately-ideally before meals to prevent nausea and after bronchodilator administration.

Pre-surgical preparation

When surgery is planned, nurses prepare children and families by explaining procedures, addressing concerns, and ensuring all preoperative requirements are met. Postoperative care focuses on pain management, respiratory monitoring, preventing complications, and gradually resuming airway clearance activities.

Patient and family education

Successful long-term management depends heavily on family involvement. The nurse educates the patient and family to avoid exposure to people with upper respiratory infections and teaches about early signs of respiratory infection so treatment can begin promptly.

Essential teaching points include proper airway clearance technique, medication administration (especially inhaled therapies), recognizing signs of exacerbation, maintaining adequate hydration and nutrition, and the importance of avoiding tobacco smoke exposure. Fighting infection and coughing can use up a child’s energy, so they may need more food than other children, with plenty of protein and vitamins to support their immune system.

Prognosis and long-term outlook

The good news is that with proper treatment, many children with bronchiectasis can lead active, fulfilling lives. In two out of three children diagnosed, bronchiectasis can be reversed or improved with the right treatment-particularly when caught early. Even when complete reversal isn’t possible, appropriate management can prevent disease progression and maintain quality of life.

Prompt diagnosis and optimal management of paediatric bronchiectasis is particularly important in early childhood, as mild bronchiectasis might be reversible at any age if treated early. This underscores the importance of not dismissing persistent wet cough in children.
