What is pediatric infective endocarditis?
Infective endocarditis involves an infection of the endocardium-the inner layer of the heart that covers the chambers and valves. When microorganisms enter the bloodstream, they can attach to damaged or abnormal areas of the heart, leading to the formation of irregular growths called vegetations. These vegetations are composed of fibrin, platelets, and infectious organisms that accumulate on valvular surfaces.
In children, the incidence of IE is estimated at 0.43 to 0.69 cases per 100,000 children annually. The condition carries significant morbidity, and nursing professionals play a crucial role in early detection, treatment administration, and complication prevention.
Causative organisms and risk factors
The most common pathogens responsible for pediatric infective endocarditis are bacteria, though fungal infections can also occur. Congestive heart failure is the most common serious complication and the leading cause of death in patients with infective endocarditis. It develops when valve destruction causes significant regurgitation, forcing the heart to work harder. Signs include increasing respiratory distress, edema, hepatomegaly, and poor feeding in infants.
Embolic events
Pieces of vegetation can break off and travel through the bloodstream, causing blockages in various organs. Cerebral emboli may result in stroke, while splenic and renal emboli cause organ infarction. Pulmonary emboli occur more frequently with right-sided endocarditis. Nurses should monitor for sudden neurological changes, flank pain, abdominal pain, or respiratory distress that may indicate embolic complications.
Prevention and family education
Educating families about infection prevention is a critical nursing responsibility. Children with high-risk cardiac conditions require antibiotic prophylaxis before certain dental procedures that manipulate gingival tissue or perforate the oral mucosa. Families should understand the importance of maintaining excellent oral hygiene and seeking prompt treatment for skin infections or dental problems.
