What is scurvy and why does it occur?
Scurvy is a clinical syndrome that develops when the body lacks sufficient vitamin C (ascorbic acid) for an extended period. Vitamin C is essential for collagen synthesis, wound healing, iron absorption, and maintaining healthy skin, bones, and blood vessels. Since humans cannot produce vitamin C internally, we must obtain it through dietary sources.
When vitamin C intake remains severely inadequate for 8 to 12 weeks, clinical symptoms begin to appear. The deficiency disrupts collagen formation, which affects connective tissues throughout the body. This leads to weakened blood vessels, impaired bone formation, and poor wound healing-the hallmark features of scurvy.
Children at highest risk
While scurvy is rare in developed countries, certain pediatric populations remain vulnerable. Infants between 6 months and 2 years who are fed evaporated or boiled milk are particularly susceptible because heat destroys vitamin C. Additionally, children with the following characteristics face increased risk:

Research indicates that nearly half of pediatric scurvy cases involve children with neurodevelopmental disorders who consume extremely restricted diets lacking fruits and vegetables.

Nursing assessment and diagnosis
A thorough nursing assessment is critical for identifying scurvy and distinguishing it from conditions with similar presentations such as leukemia, osteomyelitis, or bleeding disorders.
Comprehensive dietary history
Obtaining a detailed dietary history is essential. Nurses should ask specific questions about fruit and vegetable consumption, preferred foods, feeding challenges, and any dietary restrictions. Many cases are diagnosed after detailed dietary assessment reveals prolonged absence of vitamin C sources.
Physical examination
Physical examination should assess for skin hemorrhages, gum changes, joint swelling, and tenderness along long bones. Note that children with scurvy are often extremely irritable and resist being handled due to bone pain.
Diagnostic confirmation
While serum vitamin C levels below 0.2 mg/dL suggest deficiency, the diagnosis is often clinical. Radiographs may reveal characteristic findings including osteopenia, metaphyseal changes, and the “Frankel line” (a dense white line at the growth plate). The most definitive confirmation is rapid symptom improvement following vitamin C supplementation.

Effective nursing care for pediatric scurvy involves multiple interventions targeting symptom relief, nutritional correction, and family education.
Vitamin C supplementation
Children typically receive 100 to 300 mg of vitamin C daily for one month or until symptoms completely resolve. The route of administration depends on the child’s condition and ability to take oral medications. For children who cannot take oral supplements, intramuscular or intravenous ascorbic acid may be necessary.
Nurses should administer supplements as prescribed and monitor for the expected response. Spontaneous bleeding and systemic symptoms typically improve within days, while complete musculoskeletal recovery may take several weeks.

Pain control is a priority in pediatric scurvy management. Nurses should:
Assess pain regularly using age-appropriate pain scales. Children with developmental disorders may require modified assessment approaches.
Minimize handling during the acute phase. Even gentle movements can cause significant discomfort in children with bone and joint involvement.

Nurses document clinical improvement throughout treatment. Expected responses include decreased irritability within 24 to 48 hours, reduced bleeding within days, improved appetite and activity, and gradual resolution of musculoskeletal symptoms over weeks.
Family education for prevention
Prevention through dietary education is fundamental to nursing care. Families need practical guidance on incorporating vitamin C-rich foods into their child’s diet.
Daily vitamin C requirements
According to the National Institutes of Health, recommended daily vitamin C intake varies by age: 15 mg for children ages 1-3, 25 mg for ages 4-8, and 45-75 mg for older children and adolescents. These amounts are easily achievable through a balanced diet.
Food sources of vitamin C
Nurses should educate families about excellent dietary sources including citrus fruits (oranges, lemons, grapefruit), berries, kiwi, papaya, bell peppers, broccoli, tomatoes, and potatoes. Important teaching points include:
Fresh is best. Cooking and prolonged storage reduce vitamin C content. Raw or lightly cooked vegetables retain more nutrients.
Variety matters. Encouraging multiple vitamin C sources helps ensure adequate intake even with selective eating.
