Every baby’s first hours of life are crucial. Within the first 24 hours after birth, healthcare professionals conduct a thorough evaluation known as newborn assessment. This systematic examination helps identify normal findings, detect potential health concerns, and ensure the baby is transitioning well from intrauterine to extrauterine life. For nursing students and healthcare professionals, understanding newborn assessment is foundational to providing quality neonatal care.

Why comprehensive newborn assessment matters

The newborn period represents a significant physiological transition. The initial examination within the first 24 hours of birth is critical for determining the infant’s general well-being and identifying any red flags that may warrant further evaluation. Early detection of abnormalities allows for timely intervention, which can prevent serious complications and improve long-term outcomes.

A thorough assessment also provides an opportunity for parent education. When conducted with parents present, healthcare providers can answer questions, demonstrate normal findings, and offer anticipatory guidance about newborn care. This collaborative approach builds parental confidence and strengthens the early bond between family and healthcare team.

Initial assessment using the APGAR score

The first formal evaluation occurs immediately after delivery using the APGAR scoring system. Developed by Dr. Virginia Apgar in 1952, this rapid assessment method evaluates five key parameters: Appearance (skin color), Pulse (heart rate), Grimace (reflex irritability), Activity (muscle tone), and Respiration.

Each component receives a score of 0, 1, or 2, with a maximum total of 10 points. The assessment is performed at 1 minute and 5 minutes after birth. A score of 7 to 9 indicates good health, while a perfect 10 is uncommon because most newborns have some blueness in their hands and feet due to immature circulation.

Interpreting APGAR scores

Heart rate evaluation is considered the most critical component. A rate above 100 beats per minute scores 2 points, below 100 scores 1 point, and absent heartbeat scores 0. For respiration, strong crying earns 2 points, while slow or irregular breathing scores 1 point. Muscle tone assessment gives 2 points for active movement and 0 for flaccid muscles.

Scores below 7 indicate the baby needs immediate medical attention, such as suctioning airways or providing supplemental oxygen. If scores remain low at 5 minutes, reassessment continues at 5-minute intervals until 20 minutes. However, healthcare providers emphasize that the APGAR score doesn’t predict long-term health outcomes or neurological development.

Anthropometric measurements

Basic measurements taken shortly after birth include weight, length, and head circumference. These values help determine whether the newborn’s size is appropriate for gestational age and establish baseline data for monitoring growth.

Birth weight classification

The World Health Organization defines low birth weight as less than 2500 grams (5 pounds, 8 ounces), regardless of gestational age. This classification is significant because low birth weight infants face increased risks of health complications and require closer monitoring.

Birth weight categories include:

Normal birth weight: 2500 to 4000 grams (approximately 5.5 to 8.8 pounds). The average full-term newborn weighs about 3200 grams (7 pounds).

Low birth weight (LBW): Below 2500 grams. Babies weighing less than 1500 grams are classified as very low birth weight, while those under 1000 grams have extremely low birth weight.

Macrosomia (high birth weight): Above 4000 grams (8.8 pounds). In the United States, fetal macrosomia is often defined as birth weight exceeding 4500 grams (approximately 9 pounds, 15 ounces).

Newborns typically lose 5-10% of their birth weight during the first few days of life due to fluid loss and the transition to oral feeding. Most regain their birth weight within 10-14 days.

Gestational age assessment

Determining gestational age helps identify whether the infant is preterm, term, or post-term, and whether size is appropriate for that age. Healthcare providers commonly use the Dubowitz/Ballard Examination, which evaluates both physical and neuromuscular characteristics.

Physical features assessed include skin texture, lanugo (fine body hair), plantar creases, breast tissue development, ear cartilage, and genitalia development. Each characteristic changes predictably with advancing gestational age. The neuromuscular examination evaluates posture, arm recoil, popliteal angle, and flexibility of wrists and ankles.

When physical and neuromuscular scores are combined, healthcare providers can estimate gestational age within approximately 2 weeks of accuracy. This information guides expectations for the infant’s capabilities and potential complications.

Systematic physical examination

A thorough physical examination should be completed within 24 hours of birth. Most clinicians follow a systematic head-to-toe approach to ensure no area is missed.

Head and neck examination

The head is assessed for shape, symmetry, and abnormalities. Molding (overlapping skull bones) is common after vaginal delivery and resolves within days. The anterior and posterior fontanelles are palpated; abnormally large fontanelles may indicate hypothyroidism. Caput succedaneum (scalp swelling that crosses suture lines) and cephalhematoma (blood collection that doesn’t cross sutures) are noted birth-related findings.

Eyes are examined for red reflexes using an ophthalmoscope. Absent red reflexes may indicate cataracts, glaucoma, or retinoblastoma. Subconjunctival hemorrhages are common and resolve without treatment.

Ear position and shape are evaluated. Low-set ears may suggest genetic disorders such as trisomy 18 or trisomy 21. The mouth is inspected for cleft lip or palate, and the palate is palpated to detect submucosal clefts. The neck is examined for masses, webbing, or torticollis.

Cardiorespiratory assessment

Normal newborn heart rate ranges from 120 to 160 beats per minute, though it may decrease during sleep. The respiratory rate should be between 40 and 60 breaths per minute. Because newborn breathing is irregular, respirations must be counted for a full minute.

Heart sounds are auscultated to identify murmurs. Murmurs heard in the first 24 hours often result from patent ductus arteriosus and typically resolve within 3 days. Femoral pulses are compared with brachial pulses; weak or delayed femoral pulses suggest aortic coarctation.

The chest is observed for symmetry, retractions, nasal flaring, and grunting, which indicate respiratory distress. Lung sounds should be equal bilaterally.

Abdominal and genitourinary examination

The abdomen should appear round and symmetric. A scaphoid (sunken) abdomen suggests diaphragmatic hernia. The umbilical cord stump is inspected for number of vessels (normally two arteries and one vein). The liver edge may be palpable 1-2 cm below the right costal margin, which is normal.

In males, the penis is examined for hypospadias or epispadias, and both testes should be palpable in the scrotum. In females, the labia are inspected, and mucoid or blood-tinged vaginal discharge is normal due to maternal hormone exposure and withdrawal.

The anus is inspected for patency and proper positioning. Passage of meconium within 24-48 hours confirms gastrointestinal tract function.

Musculoskeletal examination

All extremities are examined for deformities, range of motion, and symmetry of movement. The clavicles are palpated for fractures, which may occur during difficult deliveries. The spine is inspected for signs of spina bifida, including dimples, tufts of hair, or exposed neural tissue.

Hip examination is critical for detecting developmental dysplasia of the hip (DDH). The Barlow and Ortolani maneuvers are performed with the infant relaxed. Risk factors for DDH include female sex, breech presentation, and family history. If examination is equivocal or risk factors are present, hip ultrasonography is recommended at 4-6 weeks.

Neurological assessment

The neurological examination evaluates tone, alertness, movement, and primitive reflexes. Normal newborns demonstrate flexed posture with active, symmetric limb movements.

Primitive reflexes include the Moro reflex (startle response with arm extension), rooting reflex (turning toward cheek stimulation), sucking reflex, and grasp reflex. Absence of normal reflexes or persistent hypotonia warrants further neurological evaluation.

Newborn screening tests

Beyond the physical examination, all newborns undergo screening for serious but treatable conditions. A heel-prick blood sample is collected, typically between 24-48 hours of life, and tested for metabolic, endocrine, and hematologic disorders including phenylketonuria, congenital hypothyroidism, galactosemia, and sickle cell disease.

Most states also screen for hearing loss and critical congenital heart disease. Hearing screening uses specialized equipment to assess auditory response, while pulse oximetry screening measures blood oxygen levels to detect heart defects.

Positive screening results require confirmatory testing. Early identification enables prompt treatment, which can prevent intellectual disability, developmental delays, and life-threatening complications.

Documentation and follow-up

Comprehensive documentation of all findings ensures continuity of care and provides baseline information for future assessments. Any abnormalities identified during the initial examination should be communicated to the family and followed up appropriately, whether through repeat examination, additional testing, or specialist referral.
