Keeping newborns warm through thermal regulation
Newborns struggle to maintain their body temperature independently, making thermal regulation a top priority in the first hours and days of life. Current guidelines from the American Heart Association and American Academy of Pediatrics recommend maintaining a warm environment, promptly drying the infant, and initiating skin-to-skin contact with the parent while keeping the baby’s body and head covered.
Skin-to-skin contact, also known as kangaroo care, has emerged as one of the most effective methods for temperature regulation. Research shows that this practice helps lower the risk of hypothermia, serious infections, and death, particularly in resource-limited settings. During skin-to-skin care, the baby rests chest-to-chest with the parent, wearing only a diaper, hat, and socks, with a blanket draped over their back.
How skin-to-skin contact works
The science behind kangaroo care is remarkable. Studies demonstrate that this contact helps regulate the baby’s body temperature and heart rate while promoting bonding and the release of beneficial hormones. Maternal breast temperature naturally adjusts to meet each infant’s thermal needs within just five minutes of starting skin-to-skin contact.
Research on preterm infants confirms that kangaroo care supports cardiorespiratory stability, improves sleep organization, and enhances neurodevelopmental outcomes. Even healthy full-term babies benefit from this practice immediately after birth and during the early postnatal period.
Umbilical cord care essentials
The umbilical cord stump requires careful attention until it dries and falls off naturally, typically within one to three weeks after birth. The American Academy of Pediatrics advocates for dry cord care in high-resource settings, which involves keeping the cord clean and exposing it to air or loosely covering it with clean cloth.
Best practices for cord care
Keep it clean and dry. Current AAP guidance states there is no need to use alcohol on the umbilical cord stump. Simply keep it clean, and if it becomes soiled, clean it with soap and sterile water. The diaper should be folded below the cord to prevent urine from soaking it.
Watch for infection signs. While umbilical cord infections are uncommon, healthcare providers should watch for warning signs including redness spreading around the cord base, swelling, foul-smelling discharge, or bleeding beyond a few drops. These symptoms may indicate omphalitis, a serious infection requiring immediate medical attention.
In settings where hygienic conditions are poor or infection rates are high, WHO recommends using chlorhexidine for cord cleaning to prevent infections and reduce neonatal mortality.
Monitoring for newborn jaundice
Jaundice affects approximately 80% of newborns as their immature livers work to process bilirubin, a yellow pigment produced when red blood cells break down. While most cases resolve naturally, severe hyperbilirubinemia can cause serious neurological damage if left untreated.
The AAP’s 2022 clinical practice guidelines recommend universal bilirubin screening for all newborns between 24 and 48 hours of life, or before hospital discharge if it occurs sooner. This can be done using transcutaneous bilirubin measurement or blood tests for total serum bilirubin.
Understanding jaundice risk factors
Hemolytic disease occurs when there is blood type incompatibility between mother and baby. Healthcare providers should perform blood group typing, Rh typing, and antibody screening early in pregnancy or at admission if prenatal screening was not done.
Breastfeeding-related jaundice comes in two forms. Breastfeeding jaundice results from suboptimal intake in the first days of life and typically peaks on days three to five. Supporting adequate breastfeeding within the first hour after birth and encouraging on-demand feeding at least eight times daily helps prevent this type of jaundice. Breast milk jaundice, on the other hand, occurs in well-fed infants and can persist for up to three months but is generally benign.
When to seek treatment
Visual assessment of jaundice should occur at least every 12 hours during hospitalization. Current guidelines provide specific bilirubin thresholds for phototherapy based on the infant’s gestational age, risk factors, and hours of life. For breastfed infants still jaundiced at three to four weeks of age, or formula-fed infants jaundiced at two weeks, measuring total and direct bilirubin helps identify potential pathologic cholestasis.
Establishing proper feeding techniques
Nutrition during the newborn period sets the stage for healthy growth and development. The American Academy of Pediatrics recommends exclusive breastfeeding for approximately six months, followed by continued breastfeeding with complementary foods for two years or beyond as mutually desired.
Supporting successful breastfeeding
Early initiation matters. Skin-to-skin care immediately after birth for infants delivered both vaginally and by cesarean decreases the risk of hypoglycemia and supports breastfeeding success. Delaying the baby’s first bath until 12 hours after birth allows more uninterrupted skin-to-skin contact and improves exclusive in-hospital breastfeeding rates.
Frequent feeding prevents complications. Newborns should nurse whenever they show signs of hunger, approximately 8 to 12 times every 24 hours. This frequent feeding decreases newborn weight loss, reduces the need for supplements, and lowers the risk of clinically significant hyperbilirubinemia.
Monitoring feeding adequacy
Weight loss in the postpartum period should be tracked carefully. Excessive weight loss beyond the 75th percentile for mode of delivery and infant age warrants thorough evaluation of maternal milk production, latch effectiveness, milk transfer, and signs of dehydration. Infants who continue losing weight five to six days after birth need close outpatient monitoring until weight gain is established.
Exclusive breastfeeding means the infant receives no additional foods except vitamin D or medically recommended supplements. Continuous rooming-in and avoiding unnecessary supplements support exclusive breastfeeding and help establish maternal milk supply.
Creating a nurturing environment for healthy development
Beyond meeting physical needs, newborns thrive in environments that promote emotional bonding and responsive care. WHO guidelines emphasize that families should receive information, reassurance, and support in a consistent manner from motivated health workers within a resourced and flexible health system.
Early detection of health issues requires systematic assessment at each postnatal contact. Healthcare providers should check for danger signs including difficulty breathing, inability to feed, convulsions, reduced movement, or abnormal temperature. Prompt recognition and referral of these warning signs can prevent serious complications and save lives.
Family engagement forms the foundation of successful newborn care. Parents and caregivers need education about normal newborn behavior, feeding cues, safe sleep practices, and when to seek medical attention. Building parental confidence through hands-on support and clear communication helps families provide optimal care at home.
