What is a biophysical profile?
A biophysical profile is a prenatal test that evaluates fetal well-being by examining five specific parameters of your baby’s health. Amniotic fluid volume

The ultrasound technician looks for at least one pocket of amniotic fluid that measures at least 1 centimeter across and 2 centimeters vertically. Adequate amniotic fluid is essential for your baby’s movement, lung development, and protection.
Fetal heart rate (nonstress test)
The nonstress test monitors your baby’s heart rate in response to movement. During this 20-minute test, sensors placed on your abdomen track your baby’s heartbeat. A healthy response includes at least two episodes where the heart rate accelerates by at least 15 beats per minute and remains elevated for at least 15 seconds when your baby moves. This pattern demonstrates that your baby’s nervous system is functioning properly.
When is a BPP recommended?
Healthcare providers typically recommend a BPP for pregnancies that carry higher risks or when there are concerns about fetal well-being. Common indications include maternal conditions such as hypertension, diabetes, lupus, or kidney disease. The test is also performed when pregnancy extends beyond 40 weeks, when there’s decreased fetal movement, or when previous pregnancies ended in stillbirth.
Women carrying multiples, those with too much or too little amniotic fluid, or pregnancies where the baby appears smaller than expected may also require BPP monitoring. Additionally, certain maternal factors like being over 35 years old, having obesity, being Rh-negative, or having abnormal results on other prenatal tests warrant this comprehensive assessment. For high-risk pregnancies, your provider may schedule BPPs weekly or even twice weekly during the third trimester.
How the test is performed
The procedure itself is straightforward and painless. You’ll lie on an examination table, and a technician will apply gel to your abdomen for the ultrasound portion. The ultrasound transducer moves across your belly to capture images of your baby’s movements, breathing patterns, and amniotic fluid levels. You may be able to watch these images on the screen during the examination.
For the nonstress test component, two elastic belts with sensors are placed around your abdomen. One belt monitors your baby’s heartbeat using Doppler ultrasound technology, while the other tracks any uterine contractions you might be experiencing. You might hear your baby’s heartbeat as a beeping sound or see it displayed on a monitor.
If your baby appears inactive during the test, this doesn’t necessarily indicate a problem. Babies sleep in cycles, and the technician may use techniques to encourage movement, such as using a gentle buzzer or asking you to eat or drink something. Some practitioners suggest eating before the test since babies often become more active after their mothers have eaten.
Understanding your BPP score
The scoring system for a BPP is designed to be clear and actionable. Each of the five components receives either 0 points (abnormal or absent) or 2 points (normal and present). This creates a maximum possible score of 10 points when all parameters are assessed.
A score of 8 to 10 points is considered reassuring and indicates that your baby is healthy and receiving adequate oxygen. These scores suggest normal fetal well-being, and routine monitoring can continue as planned.
A score of 6 points falls into an equivocal category, meaning the results are neither clearly normal nor clearly abnormal. In this situation, your healthcare provider will likely recommend repeating the test within 12 to 24 hours. Depending on how far along you are in your pregnancy and other clinical factors, your provider might also discuss the possibility of delivery.
Scores of 4 or below are considered abnormal and may indicate fetal distress or insufficient oxygen supply. These low scores often prompt immediate action, which could include additional testing, continuous monitoring, or delivery of your baby. Your provider will consider your gestational age, overall health status, and other factors when determining the best course of action.
Modified biophysical profile
Some healthcare providers use a modified version of the BPP as a first-line screening tool. The modified BPP combines only the nonstress test with an assessment of amniotic fluid volume, evaluating just two components instead of all five. This abbreviated version takes less time and can be performed more quickly.
If the modified BPP shows any concerning results, your provider will then perform the complete biophysical profile to gather more comprehensive information. This stepped approach allows for efficient screening while ensuring that detailed assessment is available when needed.
Clinical significance in high-risk pregnancies
The BPP plays a crucial role in managing high-risk pregnancies by providing objective data about fetal health. You may also be asked to drink fluids before the ultrasound portion to help with image quality.
Results are typically available on the same day, and your healthcare provider will discuss them with you shortly after the test.
