The first moments after birth are critical for a newborn’s health and survival. During this transition from the protected environment of the womb to the outside world, healthcare providers must act swiftly to ensure the baby adapts successfully. Immediate newborn care involves a series of essential steps that protect the infant from harm, support vital functions, and establish the foundation for healthy development.

Ensuring a clear airway
The very first step after delivery is ensuring the newborn can breathe properly. According to the World Health Organization, newly born babies who do not breathe spontaneously after thorough drying should be stimulated by rubbing the back before initiating any advanced interventions. Healthcare providers assess breathing within seconds of birth and clear any secretions from the airway if needed.
Most newborns begin breathing on their own after drying and gentle stimulation. However, routine suctioning of the nose or mouth is no longer recommended for babies born through clear amniotic fluid, as this practice can actually delay the onset of normal breathing. Only when there are specific indications, such as meconium-stained fluid or visible obstructions, should suctioning be performed.
Maintaining body temperature
Newborns lose heat rapidly after birth, making temperature regulation one of the most critical aspects of immediate care. Heat loss can occur at a rate of 0.1 to 0.3 degrees Celsius per minute in the moments following delivery. This rapid cooling happens because babies have a large surface area relative to their body weight, thin skin with blood vessels close to the surface, and minimal body fat for insulation.
The warm chain protocol
To prevent dangerous heat loss, WHO recommends a series of interventions known as “The Warm Chain”. This includes ensuring the delivery room temperature stays between 77 and 82 degrees Fahrenheit, immediately drying the baby with a warm towel within five seconds of birth, and placing a hat on the infant’s head to prevent heat loss. The wet towels must be removed quickly and replaced with dry, warm coverings.
Hypothermia in newborns, defined as a temperature below 36.5 degrees Celsius, can lead to serious complications. When babies become cold, they consume more oxygen and glucose to generate heat, which may result in low blood sugar and breathing difficulties. Cold stress also increases the breakdown of red blood cells, potentially leading to jaundice.
Assessing with Apgar scores
Within the first minute after birth, healthcare providers perform a quick but comprehensive assessment called the Apgar score. This standardized tool evaluates five key indicators of newborn health: heart rate, breathing effort, muscle tone, reflex response, and skin color. Each category receives a score of 0, 1, or 2, with a maximum total of 10 points.
The American College of Obstetricians and Gynecologists emphasizes that while the Apgar score provides valuable information about the baby’s immediate condition, it should not be used to predict long-term outcomes. A score of 7 to 10 at five minutes indicates the baby is adapting well, while lower scores signal the need for medical intervention. The assessment is repeated at five minutes and continues at five-minute intervals if scores remain below 7.
Understanding the components
Heart rate is considered the most important indicator in Apgar scoring. A heart rate above 100 beats per minute earns the maximum score of 2. Respiratory effort evaluates whether the baby is breathing well and crying vigorously. Muscle tone assesses whether the infant shows active movement or appears limp. Reflex irritability tests the baby’s response to stimulation, and appearance examines skin color, though this component has received criticism for potential bias in evaluating babies with darker skin tones.
Initiating skin-to-skin contact
Immediately after the initial drying and assessment, placing the newborn skin-to-skin on the mother’s chest provides multiple benefits. This practice, recommended for at least the first hour after birth, helps regulate the baby’s temperature, stabilizes heart rate and breathing, and promotes bonding between mother and child.
Skin-to-skin contact also supports successful breastfeeding initiation. When placed on the mother’s chest, newborns instinctively move toward the breast and begin feeding behaviors. This early contact stimulates the release of hormones that support milk production and maternal bonding.
Supporting early breastfeeding
Starting breastfeeding within the first hour of life provides essential nutrition and helps establish the mother’s milk supply. Early feeding also assists with temperature regulation and provides antibodies that protect the newborn from infection. Healthcare providers support this process by helping mothers position the baby correctly and recognize feeding cues such as rooting, hand-to-mouth movements, and increased alertness.
For babies who cannot breastfeed immediately due to medical concerns, alternative feeding methods are arranged while continuing to support eventual breastfeeding when possible.
Monitoring for respiratory distress
Healthcare providers carefully observe newborns for signs of breathing difficulties during the first hours of life. Normal newborn breathing patterns may appear irregular, with brief pauses lasting up to 10 seconds followed by rapid breathing. However, certain warning signs require immediate attention: persistent rapid breathing above 60 breaths per minute, grunting sounds with each breath, flaring of the nostrils, visible chest retractions, or blue discoloration of the lips or central body areas.
Some babies experience transient tachypnea of the newborn, a temporary condition caused by delayed clearing of lung fluid. This typically resolves within the first day of life with supportive care and monitoring.
Screening for congenital conditions
During the immediate newborn period, healthcare providers conduct visual examinations to identify any obvious congenital anomalies. This includes checking for proper formation of the face, mouth, and palate, examining the spine for any openings or unusual dimples, and evaluating the abdomen for normal organ placement. The hands and feet are inspected for extra or fused digits, and the genitals are examined for normal development.
More comprehensive screening tests, including hearing assessments and critical congenital heart disease screening using pulse oximetry, are typically performed before hospital discharge. These early detection measures allow for timely interventions that can significantly improve outcomes for affected infants.
Preventing infection
Infection prevention begins immediately after birth through proper hand hygiene and clean delivery practices. The umbilical cord stump requires special attention to prevent infection. Healthcare providers apply antiseptic agents according to local guidelines and teach parents how to keep the area clean and dry at home.
Most facilities also administer antibiotic eye ointment within the first hours of life to prevent infections that could be transmitted during delivery. Vitamin K is administered to prevent bleeding problems, as newborns have low levels of this essential clotting factor at birth.
The bigger picture of immediate care
These immediate care practices work together as a comprehensive system to support the newborn’s transition to independent life. Each intervention builds on the others: a warm baby breathes more effectively, proper positioning supports both temperature control and feeding, and early assessment allows for prompt intervention when problems arise.
The care provided in these first moments establishes patterns that continue throughout the newborn period and beyond. Parents who participate in skin-to-skin contact and early feeding often report feeling more confident in their caregiving abilities. Babies who receive optimal immediate care demonstrate better feeding patterns, more stable temperatures, and stronger parent-infant bonds.
