When pregnancy takes an unexpected turn, understanding the condition and its management becomes crucial for both healthcare providers and expectant mothers. Placenta previa is one such complication that demands careful attention, timely intervention, and a well-coordinated care approach. This condition, where the placenta partially or completely covers the cervical opening, affects approximately 1 in 200 pregnancies and presents unique challenges in obstetric management. Understanding placenta previa
Placenta previa occurs when the placenta attaches low in the uterus and covers part or all of the internal cervical opening. The condition is classified based on how much of the cervix is covered. Current guidelines classify placental location as complete placenta previa when the placenta covers the entire cervical opening, or low-lying placenta when the edge is within 20 millimeters of the cervical opening but doesn’t cover it completely.
The exact cause remains unknown, though several risk factors increase susceptibility. Women with previous cesarean deliveries, advanced maternal age over 35, multiparity, smoking, and prior uterine surgeries face elevated risk. The relationship between uterine scarring and placenta previa is well-established, as scar tissue provides an oxygen-rich environment that may attract placental implantation in abnormal locations.
Clinical presentation and diagnosis
Recognizing the warning signs
The hallmark symptom of placenta previa is painless, bright red vaginal bleeding, typically occurring after 20 weeks of gestation. Unlike placental abruption, which causes painful bleeding, placenta previa bleeding is characteristically painless and may stop spontaneously before recurring. Some women experience spotting before heavier bleeding episodes, while others may have no symptoms at all.
The bleeding often occurs as the lower uterine segment thins during the third trimester in preparation for labor. This natural thinning disrupts the placental attachment, causing blood vessels to open. Activities like sexual intercourse, vaginal examinations, or uterine contractions can trigger bleeding episodes. However, about one-third of women with placenta previa never experience vaginal bleeding during pregnancy.
Diagnostic approach
Most cases are identified during routine second-trimester ultrasound examinations. When placenta previa is suspected, particularly after vaginal bleeding, transvaginal ultrasound provides superior accuracy compared to transabdominal scanning and can be performed safely without triggering hemorrhage.
An important consideration is that early diagnosis doesn’t always mean persistent placenta previa. Nearly 90% of low-lying placentas identified in the second trimester resolve by the third trimester through a process called placental migration. This occurs as the placenta grows toward the better blood supply in the upper uterus, while the lower portion atrophies. Diagnosis should not be confirmed before 18-20 weeks gestation, and provisional diagnoses must be verified after 32 weeks or earlier if clinical circumstances warrant.
Management strategies
Initial assessment and stabilization
When a woman with known placenta previa presents with vaginal bleeding, immediate assessment is critical. Healthcare providers must establish intravenous access with two large-bore lines, initiate electronic fetal monitoring, and obtain blood work including complete blood count, type and screen, and coagulation studies. For substantial bleeding, blood should be crossed and matched for potential transfusion.
A crucial safety measure is avoiding digital cervical examinations in suspected or confirmed placenta previa cases. Such examinations can trigger torrential hemorrhage by disrupting the placenta and its vessels. Speculum examination remains safe for assessing bleeding quantity and source.
Expectant management
For stable patients before 36 weeks gestation with minimal or resolved bleeding, expectant management aims to prolong pregnancy safely. This approach includes close monitoring, activity restriction, and avoidance of activities that could provoke bleeding or contractions. Patients are typically advised against sexual intercourse, using tampons, or douching.
The decision between inpatient hospitalization and outpatient management depends on several factors: bleeding severity, number of previous bleeding episodes, proximity to the hospital, and patient reliability. If bleeding subsides for more than 48 hours and fetal status remains reassuring, outpatient management may be considered for selected patients.
For pregnancies less than 34 weeks with bleeding risk, betamethasone administration promotes fetal lung maturity. Corticosteroids may also benefit late preterm cases between 34-36 weeks in patients who haven’t previously received them.
Delivery planning
Timing and mode of delivery require careful consideration. Elective cesarean delivery is recommended at 36-37 weeks for uncomplicated cases with placenta previa. Women with risk factors such as recurrent bleeding, thick placental edge, short cervical length, or previous cesarean delivery may require earlier delivery at 36 weeks.
Cesarean section is necessary when the placenta covers the cervical opening, as vaginal delivery would cause life-threatening hemorrhage. The surgical approach should be carefully planned, with vertical skin incisions often providing optimal exposure. A high vertical uterine incision may be required if the placenta covers the lower uterine segment or if it’s underdeveloped.
Managing complications
Hemorrhage control
Postpartum hemorrhage represents the most serious complication of placenta previa. After placental delivery, the lower uterine segment’s poor contractility can result in continued bleeding. Management strategies include bimanual uterine massage, uterotonics medications, intrauterine balloon tamponade, compression sutures, uterine or internal iliac artery ligation, and arterial embolization.
In severe cases requiring immediate intervention, prophylactic measures like balloon catheter placement in uterine arteries before surgery can enable rapid blood flow reduction if hemorrhage occurs. When conservative measures fail to control bleeding, cesarean hysterectomy may become necessary to save the mother’s life.
Placenta accreta spectrum
Women with placenta previa face increased risk of placenta accreta spectrum disorders, where the placenta abnormally adheres to or invades the uterine wall. This risk escalates dramatically with previous cesarean deliveries, reaching 67% in women with five or more prior cesareans and concurrent placenta previa. Ultrasound assessment should evaluate for signs of abnormal placentation, and suspected cases require multidisciplinary planning for potential hysterectomy.
Multidisciplinary care approach
Successful management of placenta previa requires coordination among multiple specialties. The care team should include obstetricians, maternal-fetal medicine specialists, anesthesiologists, neonatologists, nursing staff, and blood bank personnel. For suspected placenta accreta spectrum, additional consultation with interventional radiologists, urologists, and general surgeons may be necessary.
Regional anesthesia, particularly spinal-epidural combination, is recommended as first-line for peripartum management in non-emergency situations. This approach reduces operative blood loss compared to general anesthesia, though conversion to general anesthesia remains possible if hysterectomy becomes necessary.
Patient counseling and outcomes
Women diagnosed with placenta previa require thorough counseling about their condition, potential complications, and management plan. Discussions should cover the possibility of blood transfusions, early delivery, cesarean section, and potential hysterectomy. Understanding warning signs that require immediate medical attention empowers patients to seek timely care.
With appropriate management, most pregnancies complicated by placenta previa result in positive outcomes. However, the condition increases risks of preterm birth, low birth weight, and neonatal respiratory complications. Maternal risks include significant blood loss, possible infertility from hysterectomy, intensive care admission, and in rare cases, death. The maternal mortality rate in developed countries remains low at approximately 0.03%, primarily related to hemorrhagic complications.
